摘 要:目的:考察131I-HAb18在正常人体内的生物学分布和器官辐射吸收剂量,为临床应用提供必需的依据。方法:一健康志愿者静脉给药后不同时间用Y相机行全身及局部显像,并采取血样本及留全尿,按医学内照射剂量(MIRD)纲要计算器吸收剂量。结果:给药后1,4,24,48,72,96及120h显像示血池,肝脏内放射性呈持续高水平,外生殖器及甲状腺亦显影。血中放射性持续清除,主要经肾由尿中排泄,给药后24h及48h分别排除给药量的28.2%及64.8%。器官吸收剂量(mGY/MBq)分别为:全身0.184,肝0.36,甲状腺3.57,睾丸1.81。有效剂量当量(EDE)为0.958mSv/MBq,受试者生命征无改变。结论:131I-HAb18用于人体是安全的,但应注意封闭甲状腺。Objective: The normal biokinetics of 131 I labeled monoclonal anti hepatocellular carcinoma (HCC) antibody HAb18 ( 131 I HAb18) was obtained and its radiation dose was calculated in a volunteer to provide essential data for clinical application of the HCC seeking agent. Methods: Serial whole body and static scintigraphic images were taken, and blood and urine samples were collected. The radiation absorbed doses to the organs were calculated according to the MIRD concept. Results: The scintigraphic images at 1, 4, 24, 48, 72, 96 and 120 hours post injection (p.i.) demonstrated a prolonged activity retention in the blood pool and high activity in the liver. The thyroid and genitals uptake was also visualized. The activity in the blood disappeared continually and excreted via urine. 28.2% and 64% of the administered activity was excreted in urine within 24 and 72 hours p.i.. The calculated organ absorbed doses, all in mGy/MBq, were as follows: whole body 0.184, liver 0.36, thyroid 3.57, testes 1.81. Effective dose equivalent was 0.958 mSv/MBq. No changes in vital signs were observed after administration of 131 I HAb18. Conclusion: Low radiation dose of 131 I HAb18 is acceptable and the radiopharmaceutical is a safe agent for detecting HCC.
