摘 要:目的探讨超分割方式加三维适形放射治疗脑瘤的价值。方法选择符合入组条件的原发性脑瘤病例60例,随机分为A组30例,B组30例。A组接受超分割方式加三维适形放射治疗,肿瘤剂量DT66 Gy^74 Gy;B组接受常规放射治疗,肿瘤剂量DT56 Gy^60 Gy,对比两组的治疗效果及副反应。结果肿瘤消退率(有效率CR+PR)两组差别显著:A组86.7%,B组60.0%,(χ2=4.176,P<0.05),1年生存率和1年复发率差别有显著性,分别为A组80%,B组50%,(χ2=4.929,P<0.05);A组10.0%,B组36.7%,(χ2=4.565,P<0.05);脑水肿发生率无显著性意义:A组36.7%,B组30.0%(P>0.05)。结论超分割方式加三维适形放射治疗脑瘤近期治疗效果优于常规放射治疗。脑肿瘤消退率和1年生存率有所提高,1年复发率降低,脑水肿发生率无增加。Objective To evaluate effect of hyperfractionated and three dimension confomml radiotherapy for brain neoplasms. Methods Sixty patients with primary brain neoplasms were randomly divided into two groups. 30 Patients in group A were treated with hyperfractionated and three dimension conformal radiotherapy to a total dose of 66 Gy-74 Gy, 30 Patients in group B were treated with conventional radiotherapy to a total dose of 56 Gy - 60 Gy, the effect and side effects of two groups were evaluated. Results The response rate of group A and group B were 86.7% and 60.0%, respectively. The one- year survival rate of group A and group B were 80.0% and 50.0%, respectively, the one - year recur rate of group A and group B were 10.0% and 30.0%, respectively, there were difference between two groups ( P 〈 0.05). For the side effects such as cerebral edema, there were not difference between two groups (36.7% in group A and 30.0% in group B, P 〉 0.05). Conclusion Hypeffracfionated and three dimension conformal radiotherapy for brain neoplasms has more effect than that of the conventional radiotherapy, the response rate and one- year survival rate were improved, one- year recur rate were reduced, there was not increasing in cerebral edema.
