96例帕金森病患者日间过度思睡的临床探讨

摘　　要：目的使用爱泼沃斯思睡量表(ESS)对帕金森病(PD)患者日间过度思睡(EDS)及其相关因素进行评价。方法对96例PD患者和98例对照进行ESS问卷、匹兹堡睡眠质量指数(PQSI)调查。PD患者病情评价使用H&Y分期、UPDRSⅢ评分。对EDS与年龄、病情、PQSI评价、病程及药物的相关性进行分析。结果ESS分值PD组为6.05±3.83,对照组为4.24±3.21;7～9分PD组18例(18.8%),对照组6例(6.1%),≥10分PD组21例(21.9%),对照组8例(8.2%),两组比较差异显著(P<0.05)。Logistic回归分析显示PD患者EDS与年龄、病程、H&Y分期、UPDRSⅢ指数、PQSI评分、左旋多巴剂量相关。结论PD患者伴发EDS常见,主要与PD本身病理改变、夜间睡眠质量及应用抗帕金森病药物有关。Objective To assess excessive daytime sleepiness （EDS） in patients with Parkinson＇s disease （PD） using the Epworth Sleepiness Scale （ESS） and to examine the main cause of EDS. Methods 96 consecutive PD patients and 98 age-matched healthy controls participated in the study. The severity of the disease was evaluated by Hoehn and Yahr stage （H＆.Y） and Unified Parkinson＇s disease Rating Scale-Ⅲ （UPDRS-Ⅲ）, and information of anti-PD medications was collected. The correlations among EDS and age, severity of PD, PQSI score, duration of illness and medications were analyzed. Results The mean ESS score was 6.05 （S. D. 3. 83） in PD patients and 4. 24 （S. D. 3.21） in controls （P〈0.01）. Eighteen patients with PD （18. 8%） had an ESS score of 7-9, whereas only six of controls （6. 1%） scored 7 9 on ESS（P〈0. 05）. Twenty-two patients with PD were found to have a score ≥10 （21.9%）, but only eight in controis （8. 2%）（P〈0.05）. ESS score was correlated with duration of illness（P〈0. 01）, H＆.Y stage （P〈0.01）, UPI）RS-Ⅲ score （P〈 0.01）, PQSI and levodopa dose （P〈0. 01）. Conclusion EDS was more common in PD patients as compared to controls, it is mainly because of neuropathological changes in the disease itself, sleep disorders during night-time, and anti-parkinsonian drugs.
