摘 要:本研究探讨中等剂量600μg/d粒细胞集落刺激因子(GCSF)对正常供者CD34+细胞的动员效果及动员前后外周血单个核细胞中T细胞亚群的变化。对2002-2004年我科31例健康供者给予rhGCSF600μg/d,在动员的第4天开始采集外周血干细胞(PBSC),并检测动员前后白细胞总数、动员后的有核细胞(NC)数、单个核细胞(MNC)数、CD34+细胞数及粒巨噬细胞集落形成单位(CFUGM),同时观察动员及采集过程中的不良反应。对12例供者GCSF动员前后外周血单个核细胞中T细胞亚群的变化进行了观察。结果显示:600μg/d组与既往使用的300μg/d组相比,rhGCSF动员后的外周血细胞中NC、MNC和CD34+细胞及CFUGM显著增加(P<0.05),受者造血重建时间缩短(P<0.05),动员和采集过程中的不良反应发生率无明显上升(P>0.05),无严重不良反应发生。动员后CD3+细胞在PBMNC中的比例较动员前下降(15.05±3.3)%,(P<0.05)。动员前后CD4+/CD8+淋巴细胞的比值无明显变化(P>0.05)。结论:600μg/d的GCSF对正常供者的动员效果好,受者造血重建快,无严重不良反应发生。动员后PBMNC中CD3+细胞的下降及CD4+/CD8+淋巴细胞比值无明显变化,从而使得移植后急性GVHD的发生率无明显上升。The study was aimed to investigate the mobilization effect of medium dose of granulocyte colony stimulating factor (G-CSF) in allogeneic peripheral stem cell transplantation and changes of T lymphocyte subgroup in PBMNC before and after mobilization. G-CSF was administered at 600μg/d ( i. e. 300μg i. v. twice a day ) for successive 5 days to 31 matched sibling or unrelated donors for the mobilization. Stem cells were harvested on the fourth day. FACS was used to analyze the NC, MNC and T lymphocyte subgroups. The results showed that the number of NC, MNC, CD34^+ cells and CFU- GM in dose of 600μg/d significantly increased (P〈0.05), compared with 300μg/d; the time for hematological reconstruction was significantly shortened ( P 〈 0.05 ) ; the ratio of adverse effects was not obviously increased ( P 〉 0.05 ) and the median percentage of CD3^+ lymphocytes before mobilization was 46.96% [ (32.36 - 57.45 ) % ] , but 40.94% [ ( 25.31 - 48.9 ) % ] after mobilizetion, while the ratio of CD4^+/CD8^+ did not significantly changed. It is concluded that the administration of G-CSF 600 μg/d in allo-PBSCT has a good effect in the mobilezation of PBSC with minor side effects, which can markedly promote hematopoietic reconstitution after transplantation. The relative amount of CD3^+ lymphocytes significantly decreased and the ratio of CD4^+/CD8^+ remained unchanged, which may lead to alleviation of a GVHD after PBSCT.
