超声引导经皮穿刺治疗肾脏单纯性囊肿方法学研究

摘　　要：目的研究超声引导经皮穿刺硬化治疗肾脏单纯性囊肿新方法.方法对1 947例(2 047个囊肿:孤立性囊肿者1 865例、2个囊肿者64例、3个囊肿者18例)单纯性肾脏良性囊肿进行超声引导经皮穿刺固化治疗,主要对固化剂选择和给药方法进行了深入研究.使用固化剂包括50%葡萄糖(32个囊肿)、鱼肝油酸钠(122个囊肿)和无水乙醇(1 893个囊肿).给药方法包括传统保留法和置换冲洗法.术后第3个月开始随访,随访时间8个月～12年.结果传统保留法50%葡萄糖组有效率81.2%(26/32)、鱼肝油酸钠组有效率89.3%(109/122)和无水乙醇组有效率93.8%(676/721);无水乙醇置换冲洗法组有效率99.8%(1 170/1 172).无严重并发症.结论超声引导经皮穿刺置换冲洗法无水乙醇治疗良性肾囊肿明显优于传统保留法无水乙醇或其他固化剂治疗良性肾囊肿.且此方法安全、有效、简便、易行,可以广泛推广应用.Objective. To study a new therapeutic method of kidney simple cyst with ultrasonically guided percutaneous puncture.Methods 2047 kidney simple cysts were treated with ultrasonically guided percutaneous punctura. Screening curing agent and medication were lucubrated. The curing agent includes 50% glucose, Sodium Morrhuate and absolute alcohol. The medication includes traditional reserve method and replacement irrg method. The patients have been followed up in 3 months postoperatively and the follow- up kept 8 months to 12 years. Results The effective rate is 81.5 % （26/32）in the group of 50% glucose traditional reserve methods, 89.3%（109/122）in the group of Sodium Morrhuate,and 93.8% （676/721）in the group of absolute alcohol. The effective rate is 99. 8%（ 1 170/1 172） in the group of absolute alcohol replacement irrg method, and no severe complication were take place. Condusion Theabsolute alcohol replacement irrg method is better than the traditional alcohol replacement reserve method and others curing agent methods on treatment of kidney simple cyst with ultrasonically guided percutaneous punctura. This method which is safe, useful, convenient, and easy to practice should be widely applicable.
