摘  要:目的评价薄层低剂量CT(LDCT)显示周围型肺癌基本征象的准确性。方法43例肺癌病灶同时接受薄层LDCT和HRCT检查,准直宽度2.5mm,层厚2.5mm,重建间隔1.5~2.5mm,球管旋转周期0.8s,薄层LDCT管电流为20~50mA,HRCT为260~300mA。分别对LDCT和HRCT所显示的肺癌征象进行分析,同时对病灶作定性诊断。结果薄层LDCT准确显示了周围型肺癌中基本征象,其中包括分叶征、毛刺征、支气管征、胸膜凹陷征和血管集中征等,并能对大部分病灶做出正确诊断,与HRCT有良好一致性(χ2=0.8,P>0.05)。结论薄层LDCT能准确显示周围型肺癌基本征象,当能做出准确定性诊断时,不必再做HRCT检查。Objective To evaluate the accuracy of thin - section low - dose CT ( LDCT ) in showing the basic characteristic signs of peripheral lung cancer. Methods 43 cases with peripheral lung cancer underwent thin - section low - dose CT and high resolution CT (HRCT), collimation 2.5 ram, thickness 2.5 mm, interval 1.5-2.5 mm, scan time 0.8 s, tube current of LDCT was 20 -50 mA and HRCT was 260 - 300 mA. 3 investigators analysed the findings of LDCT and HRCT blindly , and made a qualitative diagnosis of each case respectively . Results Thin - section LDCT could display the basic characteristic signs of peripheral lung cancer accurately including lobulation, spiealation, air bronehgram, pleual retraction, convergence of vessels and so on,and a proper diagnosis could be made through it, there was no significant difference between thin - section LDCT and HRCT(X^2 = 0.8,P 〉 0.05) . Conclusion Thin - section LDCT can display the basic characteristic signs of peripheral lung cancer, it is not necessary to perform HRCT ,if a proper diagnosis is made through it.
