选择性腰动脉栓塞全脊椎切除重建术治疗腰椎骨巨细胞瘤

摘　　要：目的探讨选择性腰动脉栓塞全脊椎切除及稳定性重建治疗腰椎骨巨细胞瘤的疗效。方法2例腰椎骨巨细胞瘤患者采用术前选择性腰动脉介入栓塞,分期手术行全脊椎切除、前后路联合重建脊柱稳定性的治疗方案。结果2例术后均获得15个月随访,神经功能恢复满意,植入椎体获得骨性融合,脊柱稳定性良好。结论选择性腰动脉栓塞下全脊椎切除、前后路联合重建脊柱稳定性治疗腰椎骨巨细胞瘤出血少,疗效满意。Objective To investigate the effect of selective lumbar arteries embolization ,total spine resection and reconstruction in the treatment of lumbar giant cell tumor of bone.Methods 2 cases with lumbar giant cell tumor of bone were undergone preoperative selective lumbar arteries embolization,staged surgery for total spine resection and anterior and postcrior reconstruction.Results 2 cases were followed up for 15 months.The surgeries were successful.The neural function recovered satisfactorily.The implanted vertebral bodies fused and the spinal stability was restored.Conclusions It requires less blood loss and can provide satisfactory result in the treatment of lumbar giant cell tumor of bone with selective lumbar arteries embolization,total spine resection and reconstruction.
