原发性回盲部恶性淋巴瘤的影像学检查

摘　　要：目的评价气钡灌肠造影和CT扫描对回盲部恶性淋巴瘤检查的效果。方法收集并分析了4例经术后病理证实回盲部恶性淋巴瘤的影像资料,其中,3例行气钡灌肠造影,1例行CT扫描。结果气钡灌肠造影显示2例回盲部黏膜被推压、拉长变细或展平,1例黏膜紊乱、粗糙僵直、交织成网格状。2例肿块单发(圆形和葫芦形)、1例多发(圆形),直径0.4cm～10.0cm,边缘光滑。2例诊断回盲部占位,1例疑诊本病。1例CT(增强)扫描见回盲部肠腔“夹心饼”样狭窄,肿块分叶界清,密度呈均匀中度增加,诊断回盲部恶性肿瘤。结论气钡灌肠造影可清楚显示回盲部黏膜改变,明确黏膜下肿物大小、数目、形态及病段肠功能变化。CT扫描可显示更有价值的征象,两种技术联合应用,可获得更多的影像信息。Objective To assess the efficacy of air-barium emena and CT scan for detecting the primary malignant lymphoma in ileocecal region. Methods The imaging data of four patients with surgically and pathologically proved primary malignant lymphoma in ileocecal region were collected and analyzed. Of them, 3 cases underwent air-barium enema examinations and 1 case underwent CT scan without and with contrast medium enhancement. Results On air-barium double contrast examination, compressed prolonged, thinned and flatted ileocecal mucosa was found in 2 cases, and disordered, coarse-rigid and intertexture-reticular ileocecal mucosa in 1. Two cases had single round or cucurbit-form masses, one case had multiple round masses with smooth margin and 0.4 - 10. 0 cm in diameter. Two cases were diagnosed as ileececal space-occupying lesions, one case was consedered as this disease. One patient who underwent CT enhanced scan presented ileocecal intraluminal “ sandwich pancake-like” stricture, clear-lobulated mass and mid-homogeneously increased density, finally, was diagnosed as malignant tumor in ileocecal region. Conclusion Air-barium double contrast examination can clearly reveal the changes of ileocecal mucosa and determine the size, number, shape of submucous masses and the functional state of affected segment. CT scan can reveal more valuable signs and therefore the integration of the both techniques can obtain more imaging informations.
