摘 要:目的探讨脑出血灶周组织及血清肿瘤坏死因子α(TNFα)含量、灶周白细胞浸润与神经元凋亡的关系。方法健康家犬30只,在数字减影全脑血管造影(DSA)介入下刺破大脑中动脉制作基底节区或颞叶脑出血模型,并分为出血后6h、12h、24h、48h、72h及96h组,每组5只。每只犬于制模前及相应时间处死前分别检测血清TNFα含量;处死后取脑检测出血灶周TNFα含量、计数浸润的白细胞及凋亡的神经元,并与健康对照组(5只)进行比较。结果模型犬脑出血灶周及血清TNFα含量分别在出血后24h和48h达峰值,两者呈正相关(r=0.914,P<0.05);出血灶周神经元凋亡及白细胞浸润均于48h达高峰,两者呈正相关(r=0.971,P<0.05),且均与灶周TNFα含量呈正相关(r分别为0.949和0.801,均P<0.05)。结论TNFα及白细胞浸润参与了脑出血后脑组织的损伤过程,两者可能与神经元凋亡有关。Objective To study the contents of tumor necrosis factor-α (TNF-α) in perihematoma and serum, and the relationship between leukocyte infiltration and neuron apoptosis. Methods 30 healthy domestic dogs were performed with DSA intervening. Middle cerebral artery was picked thus ICH model of basal segment region or temporal lobe was prepared. The experimental dogs were divided into 6 groups according to different time point after hemorrhage (6 h, 12 h, 24 h, 48 h, 72 h and 96 h) , and each group included 5 dogs. Contents of TNF-α in serum were determined before establishment of ICH model and at each corresponding time-point before execution. Brain was taken out at each time-point after execution to determine contents of TNF-α, leukocyte infiltration and expression of neuron apoptosis in perihematoma. The results were compared with control group (5 dogs). Results The contents of TNF-ot in perihematoma and in serum reached peaks at 24 h and 48 h after hemorrhage, respectively. There was a positive correlation between them ( r = 0. 914, P 〈 0. 05 ). Both expressions of neuron apoptosis in perihematoma and leukocyte infiltration reached peak at 48h. There was a positive correlation within them (r = 0. 971, P 〈0. 05) and both of them were positively correlated with the change of TNF-α contents in perihematoma ( r =0. 949 and 0. 801, respectively, all P 〈 0. 05 ). Conclusion TNF-α and leukocyte infiltration may be involved in brain injury after ICH, and they are related to neuron apoptosis,
