摘 要:目的研究环境和二十二碳六烯酸(DHA)对胎儿大脑生长发育的影响。方法选择220名21-31岁健康孕妇为试验组,从孕12周起全部检测并控制血微量元素 (铁、钙、锌、铜、碘)在正常值范围,给予充足的蛋白质饮食。孕24周起至38周给胎儿以音乐、光照和抚摸刺激,胎教+DHA组69人,从16周起至分娩每天服用1 080 MG DHA,其余151人不服用DHA为胎教组;对试验组的220名新生儿口龄12~14D和26-28D时共2次进行《新生儿20项行为神经评价》(NBNA),并与《全国12城市正常新生儿20项行为神经评价》的结果做对照。结果 12~14日龄时NBNA得40分(满分)者胎教组96.0%(145/ 151),胎教+DHA组100.0%(69/69),均高于对照组82.5%(589/714)(P<0.001); 26-28日龄时胎教组NBNA得分40分者99.3%(150/151),胎教+DHA组100.0%(69/69), 均高于对照组82.9%(592/714)(P<0.001)26-28日龄时头竖立时间≥10S者,胎教组80.8% (122/151),胎教+DHA组91.3%(63/69),均高于对照组的55.4%(342/617)(P=0),且胎教 +DHA组还优于胎教组(P<0.05);12-14日龄时看人脸的能力得4分者,胎教组51.7%(78/ 151)和胎教+DHA组78.3%(54/69),均高于对照组13.6%(59/433)(P=0),且胎教+DHA 组优于胎教组(P=0);12-14日龄注视红球能力得4分者胎教组65.6%(99/151)和胎教+DHA 组78.3%(54/69),均高于对照组13.7(71/518)(P=0)。结论胎龄满24周后每天给胎儿以听、视觉和触觉的适宜刺激,特别是附加DHA这种有利于大脑和视网膜神经元的必需脂肪酸的补给,会有利于胎儿大脑的生长发育。Objective To investigate the effect of environment and DHA on growth and development of fetus cerebrum. Methods 220 healthy pregnant women aged 21-31 years were included into triM. Beginning from 21 st week of pregnancy, their blood trace elements (iron, calcium, zinc, copper, iodine) were detected and controlled within the normal range. And all the pregnant women were on sufficient and consummate protein diet. From 24th to 38th week of pregnancy, stimulation of music, illumination and stroking were applied for their fetus. 69 pregnant women were randomized into the "prenatal influence + DHA"group, in which the pregnant women took 1 080 mg DHA per day from the 16th week of pregnancy to delivery. 151 pregnant women were randomized into the'prenatal influence" group. Neonatal behavioral neurological assessment (NBNA) were used to evaluate all the 220 babies of these pregnant women at 12-14 days of age and 26-28 days of age. The results of neonatal behavioral neurological assessments in the 12 Chinese cities were used as control group. Results The percentage of 40 score (full mark) for NBNA at 12-14 days of age in the "prenatal influence" group and the "prenatal influence + DHA" group was 96.03 % (145/151) and 100 % (69/69), respectively; both were higher than that of control group [82.5 % (589/714)](P〈0.001). The percentage of NBNA 40. 0 score at 26-28 days of age in the "prenatal influence" group and the "prenatal influence + DHA" group was 99.34 % (150/151) and 100.0 % (69/69),respectively; both were higher than that the of control group [82.9 % (592/714)] (P〈0.001). The percentage of head erect time ≥ 10 s at 26-28 days of age in the "prenatal influence" group and the "prenatal influence + DHA" group was 80.8 %( 122/151 ) and 91.3 % (63/69), respectively; both were higher than that of control group [55.4 % ( 342/617)] (P〈0.001).
