乳腺癌患者外周血乳腺珠蛋白mRNA和CD44V6-mRNA表达及临床意义

摘　　要：目的研究人乳腺珠蛋白(hMAM)mRNA和CD44v6-mRNA在乳腺癌患者外周血中的表达及临床意义。方法应用巢式-逆转录-聚合酶链反应(Nested-RT-PCR)技术检测67例乳腺癌、16例乳腺良性肿瘤患者和20例健康志愿者外周血hMAM-mRNA和CD44V6-mRNA表达。结果乳腺癌患者外周血hMAM-mRNA阳性表达率为50.7%;Ⅳ期乳腺癌患者外周血hMAM-mR-NA表达率明显高于Ⅰ、Ⅱ、Ⅲ期者(P<0.05);hMAM-mRNA在健康人外周血无表达,乳腺良性肿瘤中仅6.25%阳性表达,乳腺癌患者外周血hMAM-mRNA的阳性表达率明显高于乳腺良性肿瘤患者及健康人(P<0.05)。而CD44V6-mRNA在乳腺癌、乳腺良性肿瘤及健康志愿者中检出率差异无统计学意义(P>0.05)。结论hMAM-mRNA特异性表达于乳腺癌患者外周血,乳腺癌患者外周血中检测出hMAM-mRNA提示可能有血循环转移;而CD44V6-mRNA作为血液中乳腺癌细胞的标志缺乏特异性。Objective To explore the expression of human mammoglobin （hMAM） mRNA and CD44V6 mRNA in peripheral blood from the patients with breast cancer and its clinical significance. Methods The expression of hMAM mRNA and CD44V6 mRNA was detected in peripheral blood from 67 patients with breast cancer, 16 breast benign lesions and 20 healthy volunteers by nested reverse transcription polymerase chain reaction （Nested-RT-PCR）. Results The expression rate of hMAM mRNA was 50.7 % in peripheral blood from patients with breast cancer, and the expression of hMAM mRNA in peripheral blood of breast cancer patients with stage Ⅳ was significantly higher than those with stages Ⅰ, Ⅱ and Ⅲ （P 〈 0.05）. No hMAM mRNA expression was detected in peripheral blood of healthy volunteers, but only 6.25 % patients with breast benign lesions was positive. There was no difference in the expressions of CD44V6 mRNA in peripheral blood among breast cancer, breast benign lesions and healthy volunteers （P 〉0.05）. Conclusion hMAM mRNA is sensitive and specific for breast cancer, and the positive expression of hMAM mRNA in peripheral blood might predict the hematogenous metastatic spreading of tumor cells. CD44V6 mRNA as a marker of breast cancer cells in peripheral blood is lack of specificity.
