nm23与p21在胃癌中表达的意义

摘　　要：免疫组化研究ｎｍ２３与ｐ２１在８８例胃癌的表达变化与胃癌生物学行为及淋巴结转移的关系。结果发现ｎｍ２３低表达与胃癌侵袭程度显著相关（Ｐｅａｒｓｏｎ列联系数Ｐ＝０．２８，Ｐ＜０．０５）。ｎｍ２３低表达者淋巴结转移率（９２．７％）比正常表达者高（４８．５％，Ｐ＜０．０５）。同时发现ｐ２１阳性染色率为６５．９％，ｐ２１阳性染色与肿瘤组织分型及癌细胞增殖活性呈正相关（Ｐｅａｒｓｏｎ列联系数分别为Ｐ＝０．３８和０．３５，Ｐ＜０．０５），ｐ２１阳性肿瘤的淋巴结转移率（８７．９％）明显高于ｐ２１阴性的肿瘤（５３．３％，Ｐ＜０．０５）。ｎｍ２３低表达和ｐ２１过表达在胃癌淋巴结转移中的表现为独立的联合作用。提示ｎｍ２３低表达和ｐ２１过表达在胃癌淋巴结转移和肿瘤增殖中起重要的作用，ｎｍ２３与肿瘤浸润有关，而ｐ２１与肿瘤组织分型和增殖有关。By using immunohistochemical methods,the relation of the expression of nm23 and p21 tothe biological behavior and metastasis to the lymph nodes in the primary gastric carcinoma(in 88 cases).The results showed that a significant correlation between the lower-expresion of nm23 and the depth ofcancer inrasion was found.nm23 lower-expressive tumours were associated with a higher incidence ofmetastasis to lymph nodes (92.7%)than were nm23-normal expressive(48.5%,P<0.05).Abnormalitiesof p21 expression were found in 65.9% of the 88 primary gastric carcinoma. A significant associations wasfound between p21 over-expression and the morphologic pattern of the tumor and the proliferative activi-ties.The metastatic rate in tumours stained positively for p21(87.9%)were higher than in those with nega-tive p21 staining(53.3%,P<0.05).The contributions of nm23 lower- expression and p21 over-expressionto the lymph nodal metatasis were the independent joint action.It is suggested that nm23 lower-expressionand p21 over-expression played significant role in the metastasis to lymph nodes in the primary gastric car-cinoma.nm23 lower expression related with the cancer invasion p21 over-expression with the morphologicpatitern and proliferative activity of gastric carcinomas.
