经尿道钬激光膀胱肿瘤切除临床指标对比分析

摘　　要：目的比较经尿道钬激光膀胱肿瘤切除术(holmiumlaserresectionofbladdertumors,HOLBT)与经尿道膀胱肿瘤汽化电切术(transurethralelectrovaporiationandresectionofbladdertumors,TVBT)的临床效果。方法将膀胱肿瘤患者随机分为两组,分别行HOLBT和TVBT,记录、测定两组患者术前、术中和术后的相关临床指标,并将各指标进行统计学处理。结果术前两组一般情况比较差异无统计学意义(P>0.05),手术时间差异无统计学意义(P>0.05),HOLBT组术中出血量明显小于TVBT组(P<0.01),HOLBT组术后膀胱冲洗时间、尿管留置时间和术后住院时间均小于TVBT组(P<0.05),术后6个月复发率差异无显著性。HOLBT组术中无闭孔神经反射发生,TVBT组发生闭孔神经反射4例。结论HOLBT治疗膀胱肿瘤与TVBT疗效相近,术中并发症及术后恢复时间明显少于TVBT,值得进一步推广应用。[Objective] To compare the clinical effect of holmium laser resection of bladder tumors（HOLBT） with transurethral electrovaporiation and resection of bladder tumors（TVBT）. [Method] 59 cases of patients diagnosed bladder tumor actually were divided into two groups randomly, of them 29 cases underwent HOLBT and 30 cases TVBT. The significant markers and therapeutic results were recorded and analyzed. [Result] There was no significant difference between the two groups in some preoperative factors, time of operation and rates of recurrence of 6 months after operation. Marked difference was found between the two groups in the volume of blood loss during operation. Significant differences were found between the two groups in the mean bladder irrigating time, catheterization time, and length of hospital stay after operation. There were 4 cases obturator nerve reflection happened in TVBT group, while no obturator nerve reflection occurred in HOLBT group. [Conclusion] HOLB is as effective as TVBT, however, with less ohtainment of adverse side effect and recovering time. It should be performed extensively.
