注射用碳酸酰胺过氧化氢在头颈部鳞癌放疗中的增敏作用

摘　　要：目的 观察放射增敏剂注射用碳酸酰胺过氧化氢(CO(NH2)2·H2O·)配合放疗治疗头颈部鳞癌的疗效及放 射反应的变化。方法 对66例头颈部鳞癌患者随机分为增敏组和常规放疗组。常规放疗组33例(对照组),常 规外照射总量DT60-70Gy/6-7周,每次2Gy,每周5次,均采用6MVX线照射。增敏组33例,外照射合并注射 用内给氧,外照射同常规放疗组;注射用内给氧给药方案:生理盐水100ml+CO(NH2)2·H2O2 600mg/m2,静脉滴注 (2h),每周3次,点滴完即行放疗。结果 ①完全缓解率(CR)增敏组和对照组分别为60．61％和33．3％(x2= 4．9272 P=0．026);差异有显著性。②原发灶和颈部淋巴结转移灶达到完全缓解时放射增敏比(SER)分别为1．26 和1．20(P=0．0282)。③放射不良反应两组差异无显著性。结论 注射用碳酸酰胺过氧化氢对头颈部鳞癌的放疗 有一定的放射增敏作用,尚未发现明显毒副反应,值得进一步深入研究,长期疗效亦有待于追踪和观察。Objective： To evaluate the effect of external radiotherapy combine CO（NH2）2-H2O2 Sodium on head-neck squamous cell carcinoma and the change of the radiation reaction. Methods： 66 cases of head-neck squamous cell carcinoma were randomly divided into two groups. Radiosensitization group （RSG33 cases）：routine radiotherapy same as the conventional fraction group. The CO（NH2）2·H2O dosage regimen： NS 100ml＋CO（NH2）2·H202 600mg/m^2 ivgtt, per week 3 times; The conventional fraction group （CF33 cases） received routine radiotherapy 2Gy per day 5 times a week to a total dose DT of 60～70Gy/6～7 weeks. All patients were irradiated by 6MVX ray. Results： ① The CR：RSG and CF separate were 60.61% and 33.3% （X^2=4.9272 P= 0.026）. The CR rate of RSG group is better than CF group, there was significant difference. ②The SER of lesions and neck lymph nodes were 1.26 and 1.20 （P=0.0282）when the CR was achieved. ③In aspects of radiation reaction in two groups there was no difference. Conclusion： The injective CO （NH2）2·H2O2 can increase the radio-sensitiveness of head-neck squamous cell carcinoma with no obvious toxicity. The long-terra effect is uncertain and should be observed further.
