三维超声旋转切割法在肝脏肿瘤诊断中的应用价值

摘  要:目的研究三维超声旋转切割法在肝脏肿瘤检查中的临床应用价值。方法使用的设备是三维超声工作站,与普通超声机连接。图像采集方法:首先对要检查的肿瘤进行常规二维扫查,然后采用扇形或平行自由扫查方式匀速采集图像,将图像传入三维超声工作站,图像采集后,以肿瘤为中心进行动态旋转切割。结果85例肝脏肿瘤三维超声旋转切割法均获得成功;动态旋转切割法能更全面地显示肿瘤内及边缘不同切面的形状、结构与回声;31例强回声肿瘤及21例混合回声肿瘤三维图像明显优于二维图像;23例低回声肿瘤三维图像也优于二维图像;10例等回声或接近于等回声的肿瘤二维图像显示非常模糊,但三维图像却能很好地显示,明显优于二维图像。结论三维超声旋转切割法比常规二维超声能提供更多的诊断信息,明显优于二维超声图像;动态旋转可使肿瘤立体感强,图像逼真、直观,如同实体标本一样,在临床中具有很高的应用价值。Objective To Study the value of the rotation-cut method(RCM) under threedimensional ultrasound(3D US) in diagnosis of liver tumors. Methods Three -DUS workstation connected with ordinary US device was used. Imaging data were acquired through free hang scanning with parallel or fan scanning methods after liver tumors were examined by routine two-dimensional ultrasound (2D US). Dynamic rotation-cut was performed at the center of the tumor. Results Three-DUS RCM images were obtained successfully from all the 85 patients. The shape, structure and echo features of the liver tumor were better presented through dynamic RCM. The 3D US imaging provided more information for the diagnosis and better stereo-feeling than 2D US. The 3D US imagings were superior to 2D imaging in 31 hyperecho, 21 mixed-echo, and 23 hypoecho tumors. Ten isoecho tumors which were not showed clearly by 2D US were showed clearly by 3D US. Conclusion Three-D US RCM can provide more information for the diagnosis of liver tumors than 2D US. Three-D US imaging was much superior to 2D imaging. Dynamic rotating 3D may provide better stereo-feeling for tumors.
