埃索美拉唑三联疗法根除幽门螺杆菌感染

摘　　要：目的观察埃索美拉唑三联短程疗法根除幽门螺杆菌(Helicobacterpylori,HP)以及在十二指肠球部溃疡愈合中的疗效。方法将90例经电子胃镜检查确诊为十二指肠球部溃疡,并且经快速尿素酶试验确定为HP阳性的患者,随机分成两组:治疗组和对照组;治疗组45例,埃索美拉唑20mg每日2次,羟氨苄青霉素1.0g每日2次,克拉霉素0.5g每日2次,疗程7天;对照组45例,奥美拉唑20mg每日2次,羟氨苄青霉素1.0g每日2次,克拉霉素0.5g每日2次,治疗1周后继续以奥美拉唑20mg,每天1次,口服3周。治疗结束后4周,复查胃镜并检测HP,记录患者症状改善程度、溃疡愈合率以及HP根除情况。结果90例均完成治疗方案,治疗组溃疡愈合率和有效率分别为97.8%和100%,对照组分别为93.3%和100%,两组间愈合率和有效率差异无显著意义(P>0.05);两组HP根除率分别为88.9%和86.7%,两组间差异无显著意义(P>0.05)。结论埃索美拉唑三联1周疗法在十二指肠球部溃疡病人中有很高的HP根除率和溃疡愈合率。Objective The clinical outcome of esomeprazole-based triple therapy for the eradication of helieobactcr pylori（HP） infection and the efficacy on healing of duodenal ulcer was reviewed. Methods 90 patients with Hp infection proven by rapid urea.se test, and active duodenal ulcar by endoscopy were divided at random into two groups： the treatmeat group and the control group; 45 eases in treatmeat group were treated with esomeprazole 20mg bid and amoxicillin lg bid together with clarithromycin 0.5g bid for 7 days; 45 cases in control group were treated with omeprazole 20mg bid and amoxicillin lg bid together with clarithromycin 0.5g bid, for one week, cantinue with omeprazole 20rag qd for three weeks. The eradication rate of Hp and the healing rates of duodenal ulcer were observed by endseopy on the 28th day. After completion of treatment and recording degree of the relieves of the symptoms. Results 90 patients completed the study. The ulcer healing rates and the efficacious rates of the treatment group were 97.8% and 100%, the control group were 93.3% and 100%, there was no statistically significant diffcrence between the two groups（P〉0.05）; The eradication rate of HD in the two groups were 88.9% and 86.7%, there was no Statistically Signifieant difference between the two groups（P〉0.05）. Conclusions Esomeprazole-based triple therapy one week in the patient of duodenal ulcer have highly eradication rate of Hp tmd ulcer healing rate.
