结肠J-Pouch肛管吻合结合预防性末端回肠造口术治疗低位直肠癌

摘　　要：为了减少直肠癌前切除术术后吻合口漏和排便功能障碍的不足，采用结肠J-Pouch肛管吻合加预防性未端回肠造口术，其治疗48例，术后3年生存率和复发率与Dixon术无统计学差异，但术后生活质量明显优于Dixon术组，结果表明，该术式在TME原则下，可消除吻合口漏的发生，缓解术后排便障碍，提高生存质量，是治疗低位直肠癌较理想术式。Colonic j Pouch anal anastomosis plus preventive terminal ileostomy was conducted in an at tempt to reduce tbe occurrence of postoperative anastomotic leakage and defecation disorder after the anterior resection of rectal carcinoma. Altogetber 48 cases received the surgical operation, and postoperative 3- year survival rate and recurrence rate were not of statistical difference with that of Dixon procedure,while the life quality was better compared with that of Dixon procedure. It is suggested thai this combined proce dure,in line with the principle of TME,can eliminate the occurrence of anastomotic leakage, relieve postoperative defecation disorder, raise life quality and serve as an ideal surgical procedure in the treatment of low rectal carcinoma.
