超声介导腹腔神经节毁损对晚期胰腺癌痛病人生存质量的影响

摘　　要：目的评价超声介导腹腔神经节毁损对晚期胰腺癌痛病人的止痛作用和对生活质量的影响。方法采用超声引导经皮穿刺，于腹腔动脉干根部旁侧注入无水乙醇15ml，对96例胰腺癌病人实施腹腔神经节毁损术。疼痛视觉模拟评分（VAS）和癌症病人生活质量评分标准（QOL）比较治疗前后变化。结果VAS评分较术前明显降低（P〈0．01），疼痛治疗总有效率87．5％，显效率76.1％，随疼痛程度的减轻，病人生活质量明显改善。结论疼痛是影响胰腺癌病人生活质量的重要因素，超声介导腹腔神经节毁损能明显减轻胰腺癌性疼痛，改善病人的生活质量。AIM To evaluate the analgesic therapy of celiac ganglion destruction（CGD）guided by B- us on patients with pancreatic carcinoma（PWPO）with intractable pain and its effects on life quality. METHODS 96 cases with advanced pancreatic carcinoma accepted chemical denervation - 15 ml pure alcohol injection around celiac ganglion by ultrasound guidance. The varieties of VAS and QOL were compared between pre - therapy and post - therapy. RESULTS VAS significantly decreased after treatment（ P 〈 0. 01）, with total effective rate 87.5 %, remarkable effective rate 76. 1 %, the life quality improves as the pain relives. CONCLUSION We suggest that Ache is an important factor influencing life quality of PWPO. OGD by ultrasound guidance proves to have remarkable analgesia on decrease, and can evidently imorove life aualitv PWPO.
