摘 要:目的研究单焦一片式折叠式与非折叠式人工晶状体眼的伪调节力。方法60眼(43例)被随机分为2组,每组30眼,一组行超声乳化白内障吸出单焦一片式折叠式人工晶状体植入术;另一组行小切口白内障囊外摘出单焦非折叠式人工晶状体植入术,术后进行视远、视近前房深度检查,并在矫正屈光不正的基础上测定伪调节力。随访时间在3月以上。结果Slataper视标法测定的平均伪调节力为:折叠组(2.07±0.36)D,非折叠组(1.86±0.24)D,2组间存在显著性差异(t=2.66,P<0.01);前房深度变化为:折叠组(0.11±0.17)mm;非折叠组(0,03±0.15)mm,2组间存在显著性差异(t=1.94,P<0.05),2组在视近时前房均变浅、前移,且伪调节力与前房深度变化量呈正相关(r=0.322,P<0.05)。结论人工晶状体眼自身存在一定的伪调节力;超声乳化白内障吸出单焦一片式折叠式人工晶状体植入对眼内组织扰动小,囊袋完整,生物相容性好,伪调节力明显优于小切口白内障囊外摘出单焦非折叠式人工晶状体植入术。Objective To study the pseudo-accommodation in pseudophakic eyes between single-piece foldable and nonfoldabe monofocal intraocular lens (IOL) implantation. Methods 60 eyes (43 patients)were randomly divided in two groups,30 eyes in each group. One group underwent phacocmulsification with single-piece foldable monofocal IOL implantation, the other group underwent small incision extracapsular cataract extraction with nonfoldable monofocal IOL implantation. Postoperatively, anterior chamber depth at distant and near were examined. When the correcting lenses were worn, their powers of pseudo-accommodation were measured. The follow-up time was more than 3 months. Resuits According to Slataper method the mean pseudo-accommodative power was: foldable group ( 2.07 ± 0.36)D, nonfoldable group (1.86 ± 0.24) D; there was significantly difference between two groups (t =2.66, P 〈 0.01 ). A mean changes of anterior chamber depth was: foldable group ( 0.11 ± 0.17 ) mm, nonfoldable group ( 0.03 ± 0.15 )mm ; there was significantly difference between two groups ( t = 1.94, P 〈 0.05 ). Both two groups indicated that the anterior chamber at near became shallowed and removed forward, the pseudo-accommodation power was positively correlated with change measure of anterior chamber depth (r = 0.322, P 〈 0.05 ). Conclusion There exist a certain power of pseudo-accommodation in pseudophakic eyes. The phacoemulsification with single-piece foldable monofocal IOL implantation bothered less to intraocular tissue, kept more intact to capsular bag, tolerated more kind on explant, and pseudo-accommodation power was significantly superior to the small incision extracapsular cataract extraction with nonfoldabie monofocal IOL implantation.
