前后段联合手术治疗增生性糖尿病视网膜病变

摘　　要：目的评价晶状体超声乳化联合玻璃体切除术治疗增生性糖尿病视网膜病变的效果及其相关因素。方法自2002年4月至2004年8月对增生性糖尿病视网膜病变40例(43眼)行晶状体超声乳化联合玻璃体切除术。术中行大范围眼内光凝,必要时结合周边视网膜冷凝。部分患眼行玻璃体腔注射曲安奈德。结果43眼中视力提高者38眼(88.37%)。不变者5眼(11.63%),无一眼出现视力下降。本组病例中未见术后虹膜新生血管,未见玻璃体腔再次积血而影响视力。结论前后段联合手术治疗晚期糖尿病视网膜病变疗效好,成功率高;大范围的眼内激光,是成功的重要保证。联合玻璃体腔注射曲安奈德可减轻术后反应,更好地提高视力。Objective To access the curative effect and relative fuetors of the phacoemulsification-vitrectomy joint surgery in PDR treatment. Methods 40 subjects （43 eyes） with late PDR who attended our hospital during the period form Apr 2002 to Aug 2004 were involved into the study, The Phaeoemulsification （or infusion-aspiration） and vitrectomy were jointly practiced in 43 eyes. Panretiual laser photocoagulation （PRP） anti optional periphery cryoretinopexy was applied during the surgical procedures, Vitreous triamcinolone injection was done in some cases. Results In the 43 eyes （40 cases）, visual acuity was improved in 38 eyes and kept stable in 5 eyes, while no VA reduction had been detected yet. No post operative irido-neovascularization and vision problem caused by recurrent vitreous haemorrhage were found in our study. Conclusion The anterior-posterior joint surgical procedure was an effective PDR, treatment with high success rate. The application of PRP was an important section of a successful surgery. The vitreous triamcinolone injection wouht reduce the post-operative response and improve VA of patients,
