氪红激光治疗黄斑部脉络膜新生血管临床研究

摘　　要：目的探讨用氪红激光温热疗法治疗黄斑部脉络膜新生血管（CNV）的临床效果。方法经荧光素眼底血管造影（FFA）证实的黄斑部CNV20例（20眼），采用低能量大光斑氪红激光治疗，参数为光斑直径500～1000μm（平均600．00μm），能量70—100mW（平均87．75mW），曝光时间1—10s（平均3．108），光凝数量1～12点（平均4．80点）。随访检查视力、中心视野及FFA，时间为14—48月（平均22．75月）。结果视力：改善7眼（35．00％），稳定10眼（50．00％），下降2眼（10．00％）。治疗后视力平均为0．35，与治疗前的平均视力0．26比较虽有改善但差异无统计学意义（t=1．01，P〉0．05）。12眼进行FFA检查，显示治疗后CNV完全闭合者4眼（33．33％），CNV较治疗前面积减少、渗漏减轻者6眼（50．00％），CNV增大和渗漏增加者2眼（16．67％）。结论氪红激光温热疗法是一种町被临床选用的且行之有效的黄斑部CNV治疗方法。Objective： To investigate the clinical results of krypton red laser transpupiliary thermotherapy for patients with choroidai neovascularization（CNV） at macular region. Methods 20 cases （20 eyes） of CNV at macular region diagnosed by FFA were treated with krypton red laser with lower energy and big beam spots. Laser parameters were as follow：the diameters of beam spot varied from 500 μm to 1000 μm（average 600 gin）,the power from 70 mW to 100 mW（average 87.75 mW）,the duration from ls to 10s（average 3,10s）,beam spots from 1 to 12（average 4.80） .The visual acuity,central visual field and FFA were followed up. The follow-up period was 14- 48 months（average 22.75 months）. Resuits The visual acuity increased in 7 eyes （35.00%）;remained unchange in 10 eyes （50.00%） and decreased in 2 eyes （10.00%）. The average visual acuity of 20 eyes was 0.35 at the last follow-up time compared with 0.26 in their first diagnosis. The difference was not significant （t = 1.01 ,P 〉 0.05）. FFA showed that after the treatment CNV complete closure in 33.33 % of eyes;partial closure and reduction of fluorescein leakage in 50.00% of eyes, and CNV enlargement and increase of fluorescein leakage in 16.67 % of eyes. Conclusion Krypton red laser transpupillary thermotherapy is one of the effective therapies for GNV at macular region.
