摘 要:目的:观察加减陷胸桃承汤合参麦注射液时盲肠结扎术后ARDS大鼠模型的治疗作用,并探讨其机制。方法:选用SD大鼠为实验对象,采取盲肠结扎穿孔术造成大鼠ARDS模型,分别以生理盐水、加减陷胸桃承汤合参麦注射液(合方组)、加减陷胸桃承汤、参麦注射液预处理动物,进行对照观察。于3h、6h、12h、24h不同时间点采血检测血气分析,同时检测3h和12h组动物肺组织细胞NF-kB的表达。结果:术后各时相点假手术组PaO2、PaCO2无显著性差异,模型组随时间延长,PaO2逐渐下降,PaCO2逐渐上升。用药各组PaO2亦下降,但参麦组与合方组下降幅度偏小,与桃承组、模型组相比有显著性差异(P〈0.05);PaCO2水平模型组明显升高,与其余四组相比有显著性差异(P〈0.05),而参麦组、桃承组与舍方组PaCO2水平变化不明显,与假手术组相比无明显差异(P〉0.05)。相同时相点模型组NF-kB表达显著升高,用药各组NF-kB表达水平较之显著降低(P〈0.05-0.01),其中以合方组最为明显。结论:加减陷胸桃承汤合参麦注射液能显著升高实验动物的PaO2,降低PaCO2,减少肺组织细胞NF-kB的表达,减轻肺组织细胞的损伤。Objective: Observating the therapeutic function of rats after modified to xianxiongtaocheng decoction and shenmai injection to the ARDS after CLP and discussing the mechanism. Methods: choose SD rats as the experimental subjects. Make the rats (ARDS) models with the CLP. Then pretreating animals respectively by the physiological saline, xianxiongtaocheng decoction and shenmai injection, xianxiongtaecheng decoction, shenmai injection.and contrasting among groups.After made model at 3, 6, 12, 24 the contents of blood gas analysis of rats were dectected.and at 3, 12 hours, the apoptosis of lun tissue and nuclear factor-kB expression were measured respectively. Results: After experiments, the PaO2 and PaCO2 in the normal group had less obviously differences, with the time delaying, the PaO2 of model group grandly declining, PaCO2 grandly rising.and the PaO2 of all the groups were dropping.but the shenmai injection group and xianxiongtaecheng decoction and shenmai injection group fell less. and had significant difference compared with xianxiongtaocheng decoction group and model group ( P 〈 0.05); the PaO2 of model group increasing obviously and had significant difference compared with other four groups ( P 〈 0.05) .but the level of PaCO2 in the shenmai injection group, xianxiongtaocheng decoction group and xianxiongtaocheng decoction and shenmal injection group changed little significant difference compared with the normal group ( P 〉 0.05 ) . the NF-kB expression was increasing obviously in the model group. But the NF-kB expression of other groups had obvious fallen (P 〈 0.05-0.01 ), the most was the xianxiongtaocheng decoction and shenmai injection group. Conclusions: The xianxiong taocheng decoction and shenmai injection can highten the level of PaO2, declined the level of PaCO2 and abated the impairment level of pulmonary organizations.
