摘  要:目的探讨MCSF-1和其受体,c-fms,在二尖瓣术后心内膜炎中的作用和其对瓣膜组织在细菌种植,赘生物形成中的影响以及心脏二尖瓣术后感染性心内膜炎早期家兔心脏瓣膜MCSF-1和c-fmsmRNA的表达水平变化。方法7只家兔静脉注射5×106CFU金葡菌;9只家兔行二尖瓣手术;8只家兔同时行二尖瓣手术和静脉注射金葡菌;7只正常家兔做对照。6h后取出心脏瓣膜,提取RNA,应用RT-PCR技术测量MCSF-1和c-fmsmRNA表达水平。结果二尖瓣和三尖瓣MCSF-1mRNA在二尖瓣手术后明显的高表达;附加金葡菌注射后,二尖瓣MCSF-1mRNA表达水平进一步提高(P<0.01,与二尖瓣手术组比较),而三尖瓣却没有明显变化。相反,二尖瓣c-fmsmRNA在二尖瓣手术后明显的降低表达水平(P<0.05)。结论二尖瓣手术后感染性心内膜炎早期心脏二尖瓣和三尖瓣瓣膜有不同的MCSF-1和c-fms表达反应;瓣膜组织中MCSF-1mRNA表达水平反映巨噬细胞的聚集,增殖,分化和炎症发展状态。[ Objective] To explore the roles of macrophage colony-stimulating factor-1 (MCSF-1), and its receptor, c-fms, in the development of bacterial prosthetic valve endocarditis and to analyze their effects on valve tissue during bacterial colonization and vegetation growth. [Methods] Seven rabbits received 5×10^6 colony forming units Staphylococcus aureus i.v.; nine rabbits underwent mitral surgery; eight rabbits underwent both mitral surgery and i. v. S. aureus administration. The hearts were explanted 6 hours after the intervention and the mitral and tricuspid tissues were shock frozen in liquid nitrogen. Total RNA was isolated and mRNA abundance determined by semiquantitative duplex RT-PCR using 18S RNA as internal control. [Results] Were compared to the control group consisting of seven untreated rabbits. The nlRNA expression of MCSF-1 on both atrioventricular valves was significantly upregulated after nlitral surgery. Moreover, in the mitral valve but not in the tricuspid valve, a significant further increase in MCSF-1 mRNA was observed after additional bacteremia (P 〈0.01 compared to isolated surgery). The mRNA expression of c-fms in mitral valves was significantly downregulated only 'after isolated surgery (P 〈0.05). [ Conclusion] The mitral and tricuspid valves responded differently regarding MCSF-1 and c-fms expression in the early phase of prosthetic valve endocarditis. The expression level of MCSF-1 mRNA in heart valves seems to reflect the inflammatory status in the tissue, regarding mac,ophage accumulation, proliferation and differentiation.
