老年人胃癌的彩色多普勒超声与胃镜检查对照分析

摘　　要：目的探讨彩色多普勒超声与胃镜检查对老年人胃癌的诊断价值。方法应用彩色多普勒超声对48例老年胃癌患者空腹12 h及饮水800 ml前后检查,转动体位及探头,依次对贲门、胃底、体、窦部作连续切面扫查,观察胃癌的范围、部位、大小、血流情况并与胃镜检查对照分析。结果(1)诊断符合率:本组48例胃癌彩色多普勒超声与病理诊断符合39例,符合率81.3%。胃镜与病理诊断符合41例,符合率85.4%。(2)定位准确率:彩色多普勒超声检查定位与外科手术定位符合41例,占85.4%。胃镜检查与外科手术定位符合44例,占91.0%。同时,彩色多普勒超声能准确了解有无其它脏器的转移。结论彩色多普勒超声检查老年胃癌方法简单,可反复检查,无痛苦,无损伤,准确率较高,可作为老年人胃癌诊断的首选方法,胃镜与彩色多普勒超声检查结合应用,可为临床提供更可靠的术前治疗方案及预后评估的指导。Objective To evaluate some special manifestations of old gastric carcinoma patients on color ultrasonic （US） and compare the results with gastric endoscope.Methods Color US performed slice by slice from gastric cardiac to fundns, bedy,autmm and duodenal bulb in 48 old patients of gastric carcinoma, and compared the results with endoscope findings. Results Dgnoctic, coincidence fetes between US. gastmscopy and pathologic diagnosis wore 81.3 % and 85.4 %, respectively; Location coincidece retes between US. gastmscope and operation were 81.4 % and 91.0%. Addtionally color US can exactly find the lesions extension, siz and, metastasis. Conclusion Color US examination connected with gastric endoscope can show the extension of old gastric carcinoma, it is helpful to make the treatment policy and estimate the progression. Colodls has important value in diagnosing gastric carcinoma of aged patieats.
