摘　　要：目的:研究肝硬化患者围手术期红细胞天然免疫功能(R N IA F)的变化。方法:40例病人分为两组,30例为肝炎后肝硬化患者,10例无肝功能异常的患者作为对照组,分别于麻醉前、麻醉后、术后1d、术后3d及术后7d抽取静脉血,采用红细胞在自身血浆中天然免疫黏附肿瘤细胞的方法测定红细胞天然免疫黏附功能。结果:围手术期肝硬化患者红细胞天然免疫功能不同程度低于正常人群(P<0.05);随着肝硬化C hild A、C hild B、C hild C分级依次加重;肝硬化患者红细胞天然免疫功能麻醉前后差异无显著性(P>0.05);术后第3天R N IA F水平显著性下降(P<0.05)。肝硬化患者红细胞天然免疫黏附功能与水平相关。结论:红细胞天然免疫黏附功能可以作为判断肝病围手术期病情发展变化的指标,对判断手术后病情发展、肝功能和预后具有重要作用。Objective To investigate the changes of red blood cell nature-immune-adhesion function （RNIAF）in liver cirrhosis patients during perioperative period. Methods Venous blood was extracted from 40 patients, 30 with liver cirrhosis disease, including child A, B and C, and 10 without liver cirrhosis, before and following anesthesia, 1 day, 3 days and 7 days after operation. Suspension of red cells in self-plasma was mixed with solution of carcinoma cells from mouse ascites. A tumor cell attached with 5 red cell or 2 lymphocytes/granulocytes was counted as one rosette. The adhesion rate was calculated. Results Compared with its counterpart in healthy controls, RNIAF decreased among the patients with liver cirrhosis disease, in order of cirrhosis Child A, B and C（P〈0. 05）; Compared with the baselines, RNIAF remained unchanged significantly following anesthesia, but at the third postoperative day RNIAF levels decreased greatly（P〈0.05）; RNIAF was positively correlated with CHE in patients with cirrhosis. Conclusion RNIAF may be used as an important indicator to evaluate the severity of liver diseases during the perioperative period.
