羟基喜树碱并用丹参治疗消化系癌的疗效观察

摘　　要：本文报告羟基喜树碱(HC)—丹参(SM)方案治疗一组晚期消化系统肿瘤的临床疗效。全组31例中原发性肝癌23例、胃癌6例、结肠癌2例。皆为Ⅲ,Ⅳ期,体力状况(P.S.)在Karnofsky40分以上。一疗程总剂量HC为100mg,SM为40ml(相当于生药60克)。按全国通用疗效标准评定,结果31例中,HC—SM组获CR1例,PR6例;单用HC组仅获PR3例。治后肝癌、胃癌的生存期HC—SM组较单用HC组为长。作者认为,对病期偏晚的消化系统癌瘤不能耐受强烈化疗时用本疗法进行治疗仍可达姑息之效。Cancers of the bigestive system are the most common malignancies in this country. However, results of chemotherapy for advanced diseases have always been poor. In an attempt to improve the therapeutic effect, thirty one patients with primary liver carcinoma, gastric carcinoma and colon cancer were treated with the HC-SM and HC alone regimens. All cases entered into study were stage Ⅲ and stage Ⅳ disease , with P.S. scores above 40 , according to Karnofsky . Patients were randomly divided into two groups, namely hydroxycamptothecin alone group (HC) and HC plus salvia mit-orrhiza group(HC-SM). Dosages for a course of treatment were 100 mg hydroxycamptothecin and 60g salvia mitorrhiza. 19 cases have received one course of treatment, and 12 patients received two. Among 14 cases in HC-SM group 1 snowed CR and 6 PR. And among 17 cases in HC group 3 showed PR. The survival period was longer in HC-SM group as compared to HC group for liver carcinoma(l33.6 : 46.5 days ) and gastric cacinoma (192.3 : 107 days), but was not for colon carcinoma(119.7 :127.2 days). Side effects of hydroxycamptothecin were mild, and the combination with salvia mitorrhiza did not seem to ameliorate the toxicity of HC. In overall evaluation, authors suggested that HC-SM regimen is superior to HC alone with better therapentic effect, and was well tolerated by the cancer patients.
