癌性与结核性胸腹水中淋巴细胞亚群分布、对PHA刺激的增殖反应及产生IL—2能力的初步分析

摘　　要：为比较肿瘤免疫与炎性免疫有无本质区别,本文观察了14例癌性胸腹水中单个核细胞(MN C)亚群分布状态,同时检测了胸腹水中淋巴细胞对PHA刺激的增殖反应及产生白细胞介素I(IL—2)的能力,并与8例结核性胸水进行比较。结果发现,与结核相比,癌性胸腹水中含有大量OKM_1^+的巨噬细胞及较多的T_8^+细胞,而T_4^+细胞则明显低于结核性胸水。癌性胸腹水中淋巴细胞对PHA刺激的增殖反应及IL—2产生能力均明显低于结核性胸水者,提示癌性胸腹水中的淋巴细胞处于功能受抑制状态。对癌性胸腹水淋巴细胞功能抑制的机制也进行了讨论。加入自身胸腹水后,结核的淋巴细胞增殖反应及IL—2产生能力均有所降低,提示结核性胸水中可能含有某种抑制性因素。而癌性胸腹水中的淋巴细胞加入自身胸腹水后,对PHA刺激的增殖反应及IL—2产生能力有的升高,有的降低,且这种升高或降低随胸腹水比例的变化而变化,提示癌性胸腹水中可能既含有抑制性因素,又含有刺激性因素。We have phenotyped the mononuctear cells subpopulation and observedthe blast transformation and production of interleukin 2(IL-2) of lymphocytes under PHA stimulation in vitro in 14 cases of malignant effusions and 8 cases of tuberculous (TB) effusions. It was showed that there are much more OKM1+ macrophages, T8+ cells and much less Tt cells in malignant effutions than in TB effusions. The T4/T8 ratio was 0.93 in malignant effusions and 2.42 in TB effusions. The incoporation of 3H-TdR and the production of IL-2 of lymphocytes in malignant effusions was significantly lower than that in TB effusions. The stimulation index of lymphocyte transformation (STLT) was 21.15 in malignant effusions and 157.49 in TB effusions. The stimulation index of IL-2 activity (SITL-2) was 2.28 in malignant effusions and 4.63 in TB effusions. These results suggested that T cells in malignant effusions was inhibited. To our suprise, SILT and STIL-2 become lower after adding autologous effus-sions to lymphocyte cultures in TB cases, indicating the presence of some inhibitor(s) in TB effusions. However when autologous malignant effusions was added to the lymphocyte cultures, in 3/8 cases SILT was increased and 5/8 decreased, in 4/7 cases SIIL-2 ) was increased and 3/7 decreased. These results indicated that there might be some different immunoregula-ting factors in different malignant effusions.
