摘　　要：目的探讨高频彩色多普勒超声对乳腺导管内乳头状瘤的诊断价值。方法回顾性分析54例经手术和病理学证实的乳腺导管内乳头状瘤的声像特征和彩色多普勒频谱表现。结果54例乳腺导管内乳头状瘤的声像图特征可分为4型。导管扩张伴导管内乳头状回声型35例,不规则液性暗区中实质回声型4例,单纯性导管扩张型6例,实质结节回声型9例。超声诊断符合率为72.2%。彩色血流和多普勒频谱对导管内乳头状瘤良、恶性病变的鉴别无显著差异性。结论高频彩色多普勒超声能显示乳腺导管内乳头状瘤的直接或间接声像特征,是诊断该病的一种有效检测方法。Objective：To investigate the role of high frequency and color Doppler ultrasonography in the diagnosis of mam mary intraductal papilloma. Methods： A retrospective analysis was performed, which included the database of sonography and color Doppler spectrum of 54 patients with mammary intraductal papilloma. Mammary intraductal papilloma was verified by the operation and histopathological examinations. Results： The sonography of 54 patients with mammary intraducal papilloma were divided into 4 types： 35 cases shown duct dilatation with papillary processes,4 cases shown irregular sonolucent areas with solid echogenitic masses, 6 cases shown duct dilatation, and 9 cases shown solid masses. The diagnostic accuracy of ultrasound examinations was 72.2%. The difference was not significant for color Doppler ultrasonography in the differential diagnosis between benign and malignant intraductal papilloma. Conclusions： High frequency and color Doppler sonography can demonstrate direct or indirect sonographic characteristics of mammary intraductal papilloma. It is one of effective diagnostic tools in the diagnosis of such disease.
