腹盆腔恶性肿瘤热灌注化疗联合热疗的临床观察

摘　　要：目的探讨热灌注化疗联合局部射频透热治疗晚期腹盆腔恶性肿瘤近期疗效和毒副反应。方法52例晚期腹盆腔恶性肿瘤患者随机分为腹腔热灌注化疗联合局部射频透热治疗组(治疗组)和腹腔热灌注化疗组(对照组)。结果治疗组有效率为53.8%,对照组为26.9%,差异有显著性(P<0.05),毒副作用无显著性差异。治疗组治疗后生活质量较治疗前及对照组治疗后皆有显著性差异(P<0.05),治疗组治疗后CD+4水平、CD+4/CD+8比值较治疗前升高(P<0.05〉,与对照组相比,有显著性差异(P<0.05)。结论局部射频透热化疗适用于晚期腹盆腔肿瘤伴腹腔、盆腔、肝脏转移者,可获得较好的疗效,并且能够明显改善患者的生活质量和T细胞免疫功能。Objective Introperitoneal hyperthermic chemoperfusion is one of the effective methods for peritoneo-pelvic malignant tumors in advanced stage. This article is to discuss the recent therapeutic effect, toxin and side effect treated with introperitoneal hyperthernic chemoperfusion and local radiofrequeney thermotherapy. Methods Divide 52 cases into two groups at random,one group receives introperitoneal hyperthermic chemoperfusion and local radio （ treatment group）, the other receives introperitoneal chemotherapy（control group）. Results The effect ratio of the therapeutic group is 53. 8%, the effect ratio of the reference group is 26.8%, the difference is significant（P〈0.05）and the side-effect is no difference between two groups. The quality of life in treatment group is higher than the control group （P〈0. 05） . The CD＋ 4 level and proportionality of CD^＋4/CD^＋ 8 in treatment group are higher than that of pre-therapy after management and that of control group （P〈0.05）. Conclusion Combining introperitoneal hyperthermic chemoperfusion and local radiofrequency thermotherapy can be effective management to apply and enhance quality of life and T lymphocyte immunologic function.
