摘  要:目的:动态观察肝肿瘤CT增强时环状强化的形成,分析其临床意义。方法:应用单平面动态肝动脉造影CT(SD-CTHA)观察肝细胞癌以及肝转移癌(结肠癌)在动脉造影CT时的影像学表现。结果:肝细胞癌造影早期肿瘤内部可见点状血管影或斑片状血窦影,增强达到高峰后造影剂向周边扩散形成环状强化。肝转移癌造影早期肿瘤周边形成强化,随着时间的推移造影剂向内部扩散。测肝细胞癌和肝转移癌环状强化出现时间,用Mann-Whitney检验比较(P=0.000<0.01)。结论:SD-CTHA对环状强化的肝细胞癌与肝转移癌有诊断和鉴别诊断意义。Objective: To dynamic investigate the ring shape and flower crest shape CT enhancement in hepatic tumor. Methods: Observation of single-level dynamic computed tomography hepatic arteriagraphy(SD-CTHA) so that analysis of image of SD-CTHA in hepatocellular carcinoma and hepatic metastases. Results: Sustained enhancement was noticed in tumor center and slowly diffused to tumor circumference after maximum contrast-enhancement, and flower crest shaped contrast-enhancement was formed while contrast agent was disappeared in hepatocellular carcinoma. Diffused enhancement was showed from tumor margin to center, slow and sustained enhancement in circumference was showed while contrast agent of the tumor cen- ter was disappeared after maximum contrast-enhancement in hepatic metastases. The time-intensity of ring shape enhancement was observed by Mann-Whitney method(P=0.000〈0.01). Conclusion: SD-CTHA had diagnostic and differential diagnostic value in ring shape enhancement hepatocellular carcinoma and hepatic metastases.
