摘  要:目的:研究口腔黏膜癌变过程中HIF-1αi、NOS的表达及意义。方法:建立金黄地鼠颊囊癌变模型,应用逆转录聚合酶链反应(RT-PCR)法测定地鼠颊囊癌变过程中HIF-1αi、NOS mRNA的表达。结果:HIF-1αmRNA在重度异常增生组和鳞癌组出现表达,二者间表达阳性率(44%,68%)无显著性差异(P>0.05);其他各组均未检测到HIF-1αmRNA的表达。iNOS在异常增生组和鳞癌组异常高表达,其中轻度、中度异常增生组与鳞癌组间有显著性差异(P<0.05),重度异常增生组与鳞癌组间无显著性差异(P>0.05)。结论:HIF-1α的过度表达是黏膜癌变过程中的早期行为,早于组织学上的血管形成和浸润。HIF-1αi、NOS相互协同促进口腔黏膜癌的发生与发展,检测组织中二者的表达将成为临床监测口腔黏膜癌变进展及判断预后的重要指标。Objective:To study the expression and significance of HIF-1α and iNOS in oral carcinogenesis. Method: Establishing golden hamster buccal carcinogenesis model including normal control ( 14 ), hyperkeratosis ( 7 ), epithelial dysplasia(mild 12, moderate 7,severe 9),oral squamous cell carcinoma (OSCC)(25), we detected the expression of HIF-1α and iNOS using RT-PCR technology. Result: HIF-1α mRNA was overexpressed in the groups of severe dysplasia(44% ) and OSCC(68 % ), and there was no significant difference between them in positive expression rate( P 〉 0.05). No expression of HIF-1 was found in other groups, iNOSrnRNA was overexpressed in dysplasia and OSCC groups,while no expression in normal and hyperkeratosis groups. Significant difference was found in mild(33% ), moderate dysplasia( 71% ) and groups(80 % ) (P 〈 0.05 ), but there was no difference between the groups of severe dysplasia(77 % ) and OSCC( P 〉0.05). Conclusion: The overexpression of HIF-1 may be an early event in oral carcinogenesis,and earlier than angiogenesis and tumor invasion in histology. Furthermore, the overexpression of HIF-1 and iNOS can accelerate the malignant transformation of tumors. So the detection of the expression of them will be markers to evaluate the progression and prognosis of oral cancer.
