核转录因子抑制剂对重症急性胰腺炎的影响

摘　　要：目的探讨核转录因子(NF-κB)抑制剂对重症急性胰腺炎(SAP)的影响。方法将72只Wistar大鼠随机均分为6组sham组、SAP组、吡咯烷二硫氨基甲酸酯(PDTC)Ⅰ组(100mg/kg体重)、PDTCⅡ组(25mg/kg体重)、PDTCⅢ组(10mg/kg体重)和丹参干预组。各组于造模6h后,测定血清淀粉酶、脂肪酶、TNF-α和IL-10含量;检测胰腺细胞NF-κB激活水平及凋亡情况;对胰腺组织进行病理学评分。结果与SAP组比较,PDTC干预各组和丹参组内的血清淀粉酶、脂肪酶和TNF-α含量、胰腺组织NF-κB活性水平和病理学评分明显降低(P<0.05),胰腺细胞凋亡指数显著升高(P<0.05),血清IL-10含量无显著性变化;PDTC各组和丹参组组间相互比较,各项指标差异无统计学意义。结论应用NF-κB抑制剂可有效降低SAP血清中TNF-α的水平,而不影响IL-10含量;减少胰酶及炎症因子的持续释放;促进胰腺细胞凋亡;改善SAP病情。Objective To investigate the effect of the inhibitor of nuclear factor-κB （NF-κB） on severe acute pancreatitis （SAP）. Methods Seventy-two Wistar rats were randomly divided into 6 groups （ n = 12 in each group）, including sham operation group, SAP group, pyrrolidinecarbodithioic （PDTC） Ⅰ group （100 mg/kg）, PDTC Ⅱ group （25 mg/kg）, PDTC Ⅲ group （10 mg/kg） and radix salviae miltiorrhizae group. After 6 h of establishing the model, the contents of amylase, lipase, TNF-α and IL-10 in plasma were determined. The activation level of NF-κB and apoptosis of pancreatic cells were detected, and pancreatic tissue was scored on the basis of pathological changes. Results Compared with three PDTC groups and radix salviae miltiorrhizae group, the contents of amylase, lipase, TNF-α and IL-10 in plasma were significantly lower in SAP group （ P 〈 0.05 ）, and the activation level of NF-κB and the pathological scores of pancreatic tissue were reduced, while the apoptosis index was increased in SAP group. There was no obvious difference in every data between each PDTC group and radix salviae miltiorrhizae group. Conclusion The inhibitor of NF-κB can effectively accelerate the apoptosis process of cells, reduce the release of pancreatic enzyme and cytokine, decrease the contents of TNF-α, keep the contents of IL-10 stable and improve the therapy effect of SAP.
