摘 要:目的:研究舌中隔对染料的弥散及淋巴示踪的影响,为临床上舌癌哨位淋巴结活检时规范染料的注射方式提供依据。方法:64只成年日本大耳兔按注射部位随机均分为舌尖、舌侧缘、舌中央和舌根4组;分0.1ml、0.2ml、0.3ml和0.4ml4种美蓝注射剂量,每种剂量注射上述每个部位,每组4只,记录每次实验美蓝的弥散范围和引流第一站淋巴结检出情况;用SPSS10.0软件包、双因素方差分析和Wilcoxon秩和检验对数据进行统计学处理。结果:(1)64例中,60例检出蓝染淋巴结,总检出率为93.75%;其中24例于双侧检出,分属于部位分组中的舌尖组或剂量分组中的0.4ml组。(2)注射剂量为0.1ml舌尖组的4例均失败,舌尖部和舌其他部位注射美蓝后,淋巴结检出率存在显著差异(P<0.05)。(3)各剂量组间美蓝弥散范围存在显著差异(P<0.01),各部位组间则无显著差异(P>0.05)。结论:舌中隔可阻挡美蓝弥散,应用染色法作cN0舌癌SLNB时,应考虑舌中隔的阻隔作用。PURPOSE: The effects of lingual-septum on vital-dyes diffusing and lymphatic mapping in rabbit's tongue were studied in this study to provide information for injection of vital-dye when sentinel lymph node biopsy in tonguecarcinoma was performed clinically. METHODS: 64 adult rabbits were randomly divided into four groups according to the injection sites including the tip of the tongue(group 1),lateral tongue(group 2),center of the tongue(group 3) and base of the tongue(group 4). Each with 16 rabbits; there were four doses of methylene blue-0.1ml, 0.2ml, 0.3ml and 0.4ml for injection, and each dose was injected into the tongues of 4 rabbits in each group; then the diffusion range of methylene blue in the tongue tissue and the sites of the nodes stained blue were recorded in each experiment. Two-way ANOVA and Wilcoxon rank sum test were used for data analysis. RESULTS: (1) Blue-stained nodes were identified in 60 of the 64 rabbits, the overall success rate was 93.75%; 24 of 60 rabbits, which belonged to either group 1 with injection of 0.2ml- 0.4ml methylene or other 3 groups with injection of 0.4ml methylene, were found to have blue-stained nodes in bilateral necks.(2) Blue-stained nodes had not been found in all the 4 rabbits with injection of 0.1ml methylene in group 1; and there was significant difference between group 1 and other groups(P〈0.05 ). (3) Significant difference was founded in the diffusion ranges among different dose groups(P〈0.01 ) while there was no significant difference among different site groups (P〉0.05). CONCLUSION: The diffusion of methylene blue would be terminated by the lingual septum, it should be taken into consideration when SLNB be done on patients with cN0 tongue carcinoma.
