亲和层析微柱法测定肝癌特异性AFP

摘　　要：目的建立一种方便、经济的方法检测血清中肝癌(HCC)特异性AFP(AFP-L3)。方法用小扁豆凝集素(LCA)亲和层析柱分离血清中AFP-L3,放射免疫法测定AFP及AFP-L3。结果肝癌患者血清经LCA-Sepharose4B亲和层析柱分离和含α-甲基-D-甘露糖苷的解吸液洗脱可得到AFP-L3。以AFP-L3在总AFP中所占百分比≥15%作为临界值,本法诊断HCC的敏感性为92·7%,特异性为88·2%。结论该法特异性强、敏感性高、重复性好,且方便、经济,为临床早期诊断肝癌的有用指标。Objective To establish a convenient and economic method to determine hepatoma-specific alpha-fetoprotein （ AFP-L3 ） for diagnosis of hepatocellular carcinoma （ HCC ）. Methods AFP-L3 from serum of HCC patient was separated by a mini-column Lens culinaris agglutinin （LCA）-affinity chromatography. The levels of serum total AFP and separated AFP-L3 were detected by radioimmunoassay （RIA）. Results The hepatoma-specific ALP-L3 from serum of HCC patients was eluted clearly on the LCA-sepharose gel mini-column with a solution containing alpha-methyl-D-mannoside. The total AFP in serum was separated into three peaks （ ALP-L1, AFP-L2 and AFP-L3 ） during the elution course. The AFP-L3 was a part of total AFP. A ratio of more than 15 percent for ALP-L3 to total AFP in serum was considered as a specific marker for H CC diagnosis with higher sensitivity （92.7%） and specificity（ 88.2% ）. Conclusion The present data suggested that the assay in this study is more sensitive,repeatable,convenient. ALP-L3 is a useful marker for early diagnosis of HCC.
