摘  要:目的探讨游离脂肪块眼眶移植成活情况及脂肪成活机制。方法选用成年白兔18只,取腹股沟2块1ml脂肪块植入两侧眼眶内,观察体积、CT影像、组织及超微结构变化。结果移植后3个月体积平均减少50.97%。CT显示移植区1个月呈不均匀密度影,2个月以低密度为主,偶可见条状高密度影,3个月呈均匀低密度影。光镜观察1个月可见脂肪细胞坏死、慢性炎细胞、嗜酸细胞及多核巨细胞。2个月部分脂肪细胞聚集形成脂肪小叶。3个月脂肪细胞形态、小叶结构接近正常脂肪组织。病理图像分析显示:3个月脂肪细胞面积占单位面积百分比约为85.55%。电镜观察3个月可见新形成的脂肪细胞、分化成熟的前脂肪细胞及静止的脂肪细胞前体。结论移植后3个月脂肪块部分存活,体积稳定,为眼科临床应用提供理论依据。Objective To investigate autograft fat tissue in rabbit orbit. Methods The autograft of fat tissue was performed on 18 adult rabbits. Pedicel free fat tissue from groin fat pad were implanted into the superior lateral quadrant of rabbit orbit. The survival status of grafts was evaluated by volume measurement, computed tomograph (CT)scan, light microscope and electronic microscope observation. Results The volume of the graft to implanting value was(0. 59±0. 067)ml at the 1 st month, (0.53±0.062)ml at the 2nd month and (0.49±0.099)ml at the 3rd month with a reduction about 50.97% in 3 months after intraorbital implantation. CT showed an irregular density mass in the donor site at the 1st month,a mostly low density and a little high density at the 2nd month and a regular low density mass at the 3rd month after implantation. The ischemia, adipocytes necrosis, cystlike cavities, inflammatory cell, osinophilic cell and preadipocytes in implanted fat pad were revealed with histological observation at the 1 st month. Fatty lobule formed partly at the 2nd month,and the structure of implanted fat was almost normal at the 3rd months. Histomorphometric analysis revealed that the ratio of mean cell area of normal fat was (93. 17±1.66)% ,however,after implantation,the area ratio showed a temporary decrease and gradually increase with time and reached (85.55±6.56)% at the 3rd month,which is similar to the normal fat cell area. Active adipocytes precursors and preadipocytes were found by ultrastructural study at the 1 st month after implantation. The new-formed adipocytes,mature adipocytes and stationary adipocytes precursors were found at the 3rd month. Conclusion The autograft pedical free fat tissue can survive steadily for 3 months after implantation with the surviving volume about 50%. The orbital transplantation is feasible for clinical applications.
