摘　　要：目的:探讨贲门癌癌旁与无症状人群贲门黏膜活检肠上皮化生(IM)组织中p53和PCNA的表达状况。方法:采用免疫组织化学ABC法研究了70例贲门癌癌旁IM与320例无症状人群贲门黏膜活检IM组织中p53和PCNA蛋白的表达。结果:无症状人群贲门黏膜活检标本中IM检出率4.4%(14/320),低于贲门癌癌旁组织中的38.6%(24/70)(P<0.01)。癌旁IM组织中p53蛋白高表达率高于无症状人群IM(χ2=8.7840,P<0.01),但PC-NA蛋白阳性表达率差异无统计学意义(χ2=1.0788,P>0.05)。结论:贲门癌癌旁IM可能更易发生基因改变。Aim ： To study the expression of p53 and PCNA in intestinal metaplasia（IM） tissue adjacent to gastric cardia adenocarcinoma（GCA） and gastric cardia biopsy tissue from symptom-free subjects. Methods： Immunohistochemical analysis （ABC method） was performed to detect IM from tissue adjacent to GCA（ n = 70）and gastric eardia biopsy tissue from symp- tom-free subjects（ n = 320）. Results： The positive rate of p53 protein in tissue adjacent to GCA with IM （33%） was higher than that in cardia biopsy tissue with IM from the symptom-free subjects （0%） （P 〈 0.01 ） , and the positive rate of PCNA had no significant difference between tissue adjacent to GCA with IM and cardia biopsy tissue with IM from symptom-free subjects （63% vs 79% ） （P 〉 0.05）. Conclusion： The IM tissue adjacent to GCA is prone to genetic changes.
