血清κ/λ轻链比值测定对多发性骨髓瘤的诊断价值

摘　　要：目的探讨免疫速率散射比浊法测定血清κ/λ比值及其在多发性骨髓瘤中的临床价值。方法以多发性骨髓瘤患者25例、良性M蛋白病患者28例及健康人120例为研究对象,采用Immage特定蛋白仪测定血清κ/λ比值,并分析三组检测结果的差异。结果以κ/λ>2.4为阳性判断标准,有0.8%正常人为阳性,κ、λ型良性M蛋白病组阳性检出率分别为23.1%和26.7%,κ、λ型多发性骨髓瘤组阳性检出率分别为92.3%和100.0%。结论应用免疫速率散射比浊法采用Immage特定蛋白仪测定κ、λ轻链的灵敏度和稳定性均较高,κ/λ比值预测多发性骨髓瘤具有较高的准确性。Objective To explore the clinical value of the immune rate nephelometry （IRN）in detecting the rate of serum κ and λ,and diagnosing multiple myeloma. Methods Twenty-five cases of multiple myeloma （MM） ,28 cases of M protein sickness and 120 cases of healthy people were detected for the rate of serum κ/λ, by the Immage Protein Machine the results were analyzed. Results 0.8% of healthy people was positive, the positive rate of κ and λ type M protein sickness were 23. 1% and 26.7% respectively, the positive rate of κ and λ type multiple myeloma was 92.3% and 100% respectively. Conclusion The method of IRN has high sensitivity and stability in detecting the light chain of serum κ and λ on/with the Immage Protein Machine. The accuracy value of prognosing MM with the rate of serum κ and λ is better.
