摘 要:目的研究种植区骨皮质厚度对种植体骨界面应力分布的影响。方法建立含种植体的颌骨三维有限元模型,应用三维有限元法,研究在不同厚度的骨皮质支持下,种植区骨皮质厚度对种植体骨界面应力分布的规律。结果骨皮质厚度对种植体颈部皮质骨内的应力有较大的影响,两者呈负相关。随着骨皮质厚度的增加,种植体骨界面骨皮质内的应力逐步减小。骨皮质厚度<2 mm时种植体界面骨皮质颈部的应力值明显高于骨皮质厚度≥2mm时的应力值(P<0.05);当骨皮质厚度≥2 mm时,随着骨皮质厚度的增加,种植体界面皮质骨内的应力值虽然逐步减小,但是其变化并无显著性差异。结论种植区骨皮质厚度至少应≥2 mm,以保证其生物力学的相容性。Objective To investigate the stress distributions in the implant-bone interface related to coryical thickness around implant. Methods First, construct the three-dimensional finite element models of edentulous mandible with different thickness of cortical bone and the stress distributions in the implant-bone interface were investigated under both axial and oblique loading. Results The results of this study indicated that the relationship between the cortical thickness of mandible and stress distribution in implant-bone interface was negative, the greater thickness of cortical bone, the less was the stress concentration. And the thickness of cortical bone around implant should be at least 2 mm, or the stess of implant-bone interface may increase significantly (P 〈 0.05). Conclusion In order to obtain the long-term success of implant-prosthesis, the thickness of cortical bone around implants should be at least 2 mm.
