摘 要:目的探讨含缓释淀粉的肠内营养剂(瑞代)对高血糖重危患者的影响。方法将44例患者随机分成研究组和对照组,分别给予相同热量的肠内营养支持,研究组用瑞代,对照组用能全力,观察肠内营养支持前、支持后第5天、第10天患者空腹血糖(FBG)、餐后2 h血糖(2h BG)、糖化血红蛋白(GHb)、血清甘油三酯(TG)、血清总胆固醇(TC)、血清总蛋白(TP)、血清白蛋白(ALB)的变化。结果在营养支持后第10天,研究组FBG和2h BG水平明显低于支持前和对照组(P<0.05);与支持前和对照组比较,研究组GHb浓度虽有下降,但差异无显著性;与支持前比较,研究组和对照组血清蛋白水平明显升高(P<0.05);两组间血清蛋白水平比较差异无显著性。两组患者肠内营养支持后血脂变化无显著性。结论含缓释淀粉的肠内营养剂(瑞代)能够降低高血糖危重患者的空腹血糖和餐后血糖,同时改善患者的营养状况,在临床上比标准配方更适合高血糖危重患者。Objective To study the effect of enteral nutrition with slow-release starch in critically ill patients with hyperglycemia. Methods 44 patients were randomly divided into control group and study group. Two groups received equal daily calorie in enteral nutrition. The control group was given Nutrison and the study group was given Fresubin^TM diabetes. Fasting blood glueose(FBG), postprandial blood glueose(PBG), glycosylated hemoglobin(GHb), blood lipid and serum protein were measured on the 5th and the 10th day before and after study. Results FBG and PBG values on the 10th day were lower significantly in patients who had received FresubinTM diabetes as compared with the control group and also with those before the study. The changes of GHb were not significant in patients who had used Fresubin^TM diabetes and also not significant between groups. The changes of the serum protein were significant after study. The changes of serum lipid were not significant after study and also were not significant between groups. Conclusion FresubinTM diabetes has better glyeemie control and improve nutritional status in critically ill patients with hyperglycemia. FresubinTM diabetes is more suitable for critical eases with hyperglycemia than standard enteral formula.
