轻度认知功能损害患者的听觉词语记忆特点

摘　　要：目的比较轻度认知功能损害与轻度阿尔茨海默病患者的听觉词语记忆特点。方法轻度认知功能损害组72例,轻度阿尔茨海默病组45例,对照组62例。听觉词语记忆测定采用中文版听觉词语记忆测验方法。结果与对照组[(30.2±5.6)分,(3.4±1.9)分]比较,轻度阿尔茨海默病组[(9.8±4.1)分,(2.0±1.2)分]及轻度认知功能障碍组[(1 6.4±5.5)分,(2.6±1.7)分]前5次回忆之和与学习能力均受损,组间差异有统计学意义(t=2.26,P<0.05;t=6.84,P<0.01)。轻度认知功能损害组各次回忆的语义串联与对照组的差异有统计学意义。轻度阿尔茨海默病组[(0.1±0.2)分]长延迟回忆的语义串联与轻度认知功能损害组[(0.3±0.6)分]的差异有统计学意义(t=3.11,P<0.01)。结论轻度认知功能损害患者出现了与轻度阿尔茨海默病患者类似的听觉词语记忆损害。Objective To analyze the characteristics of auditory verbal memory impairment in mild Alzheimer＇s Disease （AD） and Mild Cognitive Impairment （MCI）. Methods Auditory verbal memory test was performed in 72 patients with MCI, 45 patients with mild AD, and 62 normal controls. Results Significant intergroup differences were found in total former five free recall and learning scores. The MCI subjects（16.4± 5.5,2.6± 1.7）performed significantly more poorly than the normal control subjects（NC） （30.2±5.6,3.4±1.9）, and mild AD categories （9.8±4. 1,2.0±1. 2） showed lower results than the MCI subjects（t= 2.26, P〈0.05 and t = 6.84, P〈0.01）. Short delayed recall and long delayed recall were much lower than short memory in MCI subjects or patients with mild AD. Total recall curve showed no significant difference between MCI and mild AD categories. As compared to the NC subjects, patients with MCI showed a lower semantic clustering strategy across all recalls. Semantic clustering strategy scores at delayed recall remained the same in NC subjects, but descended fast in MCI subjects. Semantic clustering strategy curve across all recalls showed no significant difference between MCI and mild AD categories. Conclusions Patients with MCI show the same auditory verbal memory impairment as the patients with mild AD.
