高龄结肠癌的临床特点与围手术期处理

摘　　要：目的探讨高龄结肠癌外科治疗与围手术期处理的有关问题。方法分析1994年1月至2005年5月外科治疗70岁以上高龄结肠癌113例患者的临床资料。结果根治性手术切除71例(62.8%),姑息性手术切除36例(31.9%),单纯结肠造瘘及捷径手术6例(5.3%),术后有21例(18.6%)出现各种并发症,围手术期死亡2例(1.8%)。结论高龄不应视为手术禁忌;重视合并症的诊断及治疗,加强围手术期的监护与防治术后并发症是降低手术风险的关键。Objective To probe surgical treatment and perioperative management of colon carcinoma in old people. Methods One hundred and thirteen cases of elderly patients aged over 70 who received operation for colon carcinoma from Jan 1994 to May 2005 were analyzed retrospectively. Results Seventy and one cases （62.8 % ） had radical resection and 36 cases （ 31.8 % ） had palliative resection. Six cases （ 5.3 % ） experienced simple colostomy or bypass. Postoperative complications were happened in 21 cases （ 18.5 % ）, of 2 cases died during perioperation period. Conclusion Aged colon carcinoma patients should not be regarded as contraindication for surgical operation. The key of reducing operative injury was intensive monitoring and preventing and curing for postoperative complications.
