多层螺旋CT判断胃癌胃壁浸润深度的价值

摘　　要：目的评价多层螺旋CT(MSCT)术前判断胃癌胃壁浸润深度的效果。方法58例经胃镜证实的胃癌患者(T1至T4)在术前接受MSCT扫描。每个病例都根据CT图像进行胃壁浸润深度的判断,并将诊断结果与术后病理结果进行比较。结果CT的胃壁浸润深度诊断准确率为66%(33/50例),其中T4判断准确率较高,为82%(19/23例);T2、T3的判断准确率相对较低,而T1无法单独依靠CT图像进行诊断。结论MSCT对判断进展期胃癌胃壁浸润深度很有价值,但对早期胃癌的诊断仍必须综合其他的检查手段。Objective To evaluate the effect of multislice CT （MSCT） in the diagnosis of invasion depth of gastric cancer before operation. Methods Fifty-eight patients with gastric cancer diagnosed by endoscopy were performed CT scan. Everyone was diagnosed of invasion depth according to CT images. After surgery, pathologic findings were compared with CT findings. Results Regarding T staging, 33 （66%） of 50 lesions were diagnosed correctly; The accuracy of T4 staging was relatively as high as 19 of 23 lesions （82%）, while that of T2 or T3 staging were relatively low, and T1 staging can not be diagnosed only by CT findings. Conclusion MSCT is valuable for diagnosing invasion depth of advanced gastric cancer, but the diagnosis of early gastric cancer should be combined with other examinations.
