“冷”超声乳化行透明晶状体摘出及折叠式人工晶状体植入术治疗超高度近视

摘　　要：目的观察“冷”超声乳化行透明晶状体摘出联合低或负度数后房型人工晶状体植入术治疗超高度近视的效果。方法对36例（68眼）超高度近视患者施行透明晶状体“冷”超声乳化联合低或负度数折叠式后房型人工晶状体植入术，随访3～6个月，观察术后效果和屈光状态及并发症。结果术后裸眼视力均大于或等于术前矫正视力，术后裸眼视力〉0．5者42眼，术后矫正视力〉0．5者54眼；术前角膜内皮细胞计数为（1640～2600）·m^-2，平均为（1890±210）·m^-2；术后为（1540～2480）·m^-2，平均为（1820±180）·m^-2；与术前比较差异均无显著意义（P〉0．05，x^2=1．578）。随访中无视网膜脱离及黄斑囊样水肿等并发症发生。结论“冷”超声乳化行透明晶状体摘出及折叠式低或负度数人工晶状体植入术，对超高度近视是一种安全、有效的增视手术。[眼科新进展2006；26（3）：212—213，215]Objective To observe the effect of ＇cold＇ phacoemulsification with hyalo-lens removal and low or negative power posterior chamber foldable IOL implantation in treating extremely high myopia. Methocls ＇ Cold＇ phacoemulsification with hyalocataract removal and low or negative power posterior chamber foldable IOL implantation were performed on 36 cases（68 eyes） diagnosed with extremely high myopia, after 3--6 months follow-up, observed the effect, status of refraction and complications. Results After surgery, all eyes had same naked visual acuity or better,42 eyes had a naked visual acuity of 0.5 or better,54 eyes had a corrected vision of 0.5 or better; Preoperative corneal endothelial cell count were （1 640-2 600） ·mm^-2, （1 890 ± 210） ·mm^-2 in average, postoperative were（ 1 540--2 480）·mm^-2, （ 1 820 ± 180）·mm^-2 in average. There was no detachment of retina, CME or other complications found among follow up patients. There were no significant difference between preoperative and postoperative cell counts （P 〉0.05, x^2=1.578）.Conclusion ＇Cold＇ phacoemulstfication with hyalo-cataraet removal and low or negative power posterior chamber foldable IOL implantation is a safe and effective method to improve visual acuity of extremely high myopia.
