摘  要:目的在兔离体灌注心脏观察选择性钠氢交换抑制剂HOE694对内源性自由基所致心功能损伤的保护作用。方法离体兔心脏采用Langendorff法灌流,通过次黄嘌呤(HX)和黄嘌呤氧化酶(XO)诱导产生氧自由基超氧阴离子(O2-)造成心功能损伤模型,记录左室内压(LVP)、左室内压最大上升速率(+dp/dtmax)、左室舒张末压(LVEDP)、心率(HR),定时收集冠脉流出液测量冠脉流量(CF)和肌酸肌酶(CK)活性,并测定心肌组织中丙二醛(MDA)的含量。结果用含0.06μmoL/L HX和2 U/L XO的K-H液灌流心脏10 min,可显著降低LVP、+dp/dtmax,升高LVEDP,减慢HR,增加CK释放和MDA的含量;预先灌注HOE694(0.1,0.3μmoL/L)可明显促进HX+XO所致心功能损伤的恢复,减少CK的释放和MDA的含量。结论HOE694对内源性自由基所致心功能损伤具有明显保护作用。Objective To observe the protective effects of the specific Na+/H+ exchange inhibitor HOE694 on myocardial injury induced by endogenous free radical in isolated rabbit hearts. Methods The isolated rabbit hearts were perfused in a Langendorff model. The hearts were perfused with xanthineoxidase (XO,2 U/L)and hypoxanthine (HX,0.06 mmoL/L)to induce myocardial injury. Left ventricular pressure (LVP), the frist derivative of LVP ( + dp/dtmax), left ventricular end-diastolic pressure(LVEDP)and heart rate (HR)were recorded. Coronary flow (CF)and the activity of creatine kinase(CK)in coronary effluent were measured by timed collection of coronary effluent. At the end of the reperfusion, the content of malondialdahyde(MDA)in cardiac tissue was measured. Results Perfusion with HX+XO for 10 min caused a significant impairment of cardiac function, as shown by a decrease in LVP and +dp/dtmax,and an increase in LVEDP, the relase of CK and the content of MDA. Pretreatment with HOE694 (0.1, 0.3μmoL/ L)for 10 min produced a significant improvement of cardiac function and a decrease in the release of CK and the content of MDA. Conclusion HOE694 protects heart against injury induced by HX + XO in isolated rabbit hearts.
