去除白细胞降低地中海贫血患者输血反应的临床研究

摘　　要：目的 探讨去除白细胞对添加剂红细胞质量影响及去除白细胞输血降低地中海贫血患者输血反应的效果。方法 对白细胞过滤前后的添加荆红细胞检测红细胞（RBC）、白细胞（WBC），及血红蛋白（Hb）、钾离子（K^＋）浓度、游离血红蛋白细菌生长状况，观察分析2002年1月-2005年7月间20例地中海贫血患者使用未过滤及过滤添加剂红细胞后的输血状况。结果 过滤后的添加剂红细胞白细胞去除率99．99％，红细胞回收率96．83％，其它指标去白细胞过滤前后无统计学差异（P〉0．05）；地中海贫血患者使用过滤的添加荆红细胞非溶血性输血反应发生率为0％，较应用前24．8％明显降低，两者比较有显著性差异（P〈0．01）。结论 白细胞过滤器能高效地去除白细胞，对红细胞性质功能影响极少且能有效降低地中海贫血患者输血反应。Objective To investigate the clinical effect of decreasing transfuion reaction through filtrating leukocyte in patients with Mediterranean anemia. Methods Red blood cell,white blood cell,Hemoglobin,the concentration of patassium and bacteria growth expession in free hemoglobin were detected before the blood was filtrated and after the blood was filtrated. The transfusion reaction of 20 cases with Mediterranean anemia were studied after they used non-filtrated leukocyte added Red blood cell or filtrated leukocyte added Red blood cell. Results The leaching rate of leukocyte in added Red blood cell was 99.99%. The retrieve rate of erythnocyte was 96.83%. There was non-significance of the other index between the erythanocyte was prefiltrated and profilerated（P〉0.05）. The incidence of nonhemolytic transfusion reaction was zero after the cases with mediterranean anemia were intromitted filtrated erythrocyte. The incidence of the non hemolytic transfusion reaction was 24.8% when the case with mediterranean aremia were intromitted non-filtrated erythrrocyte. The difference was significant （P〈0.01）. Conclusion Leukocyte filtrate could remove leukocyte but hardly affect the function oferythrocyte and significantly decrease transfusion reaction of the case with mediterranean anemia.
