摘 要:目的:了解周围型肺癌患者外周血血浆中p16基因异常甲基化发生情况及其在周围型肺癌临床辅助诊断中的应用价值。方法:选取56例肺周围型结节手术患者的血浆和组织标本,其中31例为周围型肺癌,25例为肺部良性结节,采用甲基化特异性聚合酶链反应方法检测p16基因启动子的异常甲基化情况。结果:在周围型肺癌患者的血浆和肿瘤组织中,p16基因甲基化检出率分别为64.5%(20/31)和70.9%(22/31);周围型肺癌患者血清、肿瘤组织中p16基因的甲基化异常事件与肿瘤分期、分型无明显相关性。在25例良性肺部疾病患者中,3例血浆和手术切除标本检测出p16基因启动子的异常甲基化存在;良、恶性肺周围型结节的p16基因异常甲基化发生情况差异存在显著性。结论:检测周围型肺癌患者血浆中p16基因异常甲基化情况有可能成为有价值的临床辅助诊断手段。Objective To detect the hypermethylation of p16 gene in plasma DNA from patients with peripheral lung cancer and its clinical value on the diagnosis of peripheral lung cancer. Methods Using a methylation-specific polymerase chain reaction, the status of methylation of the p16 gene was investigated in plasma DNA and matched pathological tissues excised in operation from 31 cases of peripheral lung cancer and 25 cases of benign lung nodi patients. Results Hypermethylation of the p 16 gene was presented in 20 cases of the plasma samples and 22 cases of the tumor tissues from 31 pateints with peripheral lung cancer but in only 3 cases of plasma and diseased tissues from 25 patients with benign lung diseases(64.5% and 70.9% vs 12.0%, P 〈 0. 05 and 0, 05, respectively), However, hypermethylation of the p16 gene in plasma and the corresponding tumor tissues weren't signitlcandy correlated with the clinical stage and pathological type of the tumor. Conclusion It is suggested that the detection of the hypermethylation of p16 gene may be a alternative and useful technique in the diagnosis of peripheral lung carcinoma.
