立体定向放射治疗胰腺癌的近期疗效

摘　　要：目的：研究立体定向放射治疗胰腺癌（PC）的近期疗效。方法：采用立体定向伽玛射线全身治疗系统治疗305例PC患者。根据肿瘤的位置、临床靶体积、病人的身体状况与治疗目的，制定放射治疗计划及调整剂量分布。等剂星曲线50—80％．周边照射总剂量3000—4000cGy，分割处方剂量400—520cGy，重复治疗6—8次，隔日治疗。结果：治疗结束后3—12个月，中位随诊时间6．23个月。115例治疗前梗阻性黄疸者，消退78例（67．8％）。198例治疗前伴腹背部疼痛，完全消失121例（61.1％），疼痛缓解55例（27．8％），22例无效（11．1％），止痛有效率为88．9％。179例病人复查CT或MR，肿瘤消失41例（22．9％）。缩小101例（56．4％），无变化21例（11．7％），增大16例（8．9％）。121例治疗期问出现轻度副反应，但可继续完成治疗。死亡91例（29．8％）。结论：立体定向放射治疗Pc对周围组织器官伤害轻微，具有定位精确、治疗时间短、副作用小的特点，近期疗效较为满意。Objective： To investigate the short term therapeutic effectiveness of stereotactic radiotherapy in the patients with pancreatic carcinoma（PC）. Metbods： The therapy of 305 patients with PC with Stereotactic Gamma Ray Body Therapeutic System. To work out radiotherapeutic plan and set dosage distribution on the basis of the carcinoma position, clinical target volume, patient health condition, and therapeutic ai,n. The isodosage curve was 50-80%. The peripheral radiation total doses was 3000-4000cGy and the fractional doses was 400-520cGy. The therapy had been repeated for 6-8 times, two days per time. Results： The mean-time of follow up was 6.23 months.Mter the therapy 3-12 months 78 patients （67.8%） recovered from theirs obstructive jaundice among 115 patients. There were 198 patients with＂dorsoventral pain before the therapy.121 patients （61.1%） got complete recover, 55 （27.8%） pain remission, and 22 （11.1%） ineffective. The analgesic effective rate was 88.9%. 179 patients received CT, or MR test. The carcinoma disappeared in 41 （22.9%） patients, shrank in 101 （56.4%）, no varied in 21 （11.7%）, and augmented 16 （8.9%）. 121 patients had mild side reaction, but could continue having therapy. 91 （29.8%） patients died after from other causes. Conclusion： There is only mild damage on surrounding tissue and organs on the therapy of PC with stereotatic radiotherapy, which has accurate localization, short period of therapeutic time, and mild side reaction. The short term therapeutic effectiveness of PC with stereotatic radiotherapy is satisfactory.
