摘 要:目的探讨耳声发射、中耳声导抗、ABR相结合的综合听力评估方法,在学龄前儿童的临床应用。方法选择120例无法配合行为测听的儿童,检测瞬态诱发耳声发射、畸变产物耳声发射以及中耳声导抗,结合短声以及有频率特异性的短纯音刺激的ABR测试来评估听力。结果120例受检者中,上述3项检查都通过者51例,诊断为感音神经性聋64例,混合性耳聋2例,传导性耳聋1例,神经性耳聋1例,听神经病1例。结论采用有频率特异性的短纯音刺激的ABR,结合耳声发射、中耳声导抗能较好地评估5岁以下无法配合行为测听的儿童的听阈。Objective To explore the clinical application of audiologic evaluation in children who failed the behavioral hearing test, by using the auditory brain stem responses (ABR), transient evoked otoacoustic emission (TEOAE), distortion product otoacoustic emission (DPOAE) and middle ear impedance measure. Methods One hundred and twenty infants and young children who failed the behavioral hearing tests were studied. TEOAE, DPOAE and middle ear impedance measure were used to assess the function of middle-ear. ABR threshold testing stimulated by click was employed to assess high-frequency hearing, and toneburst to lowfrequency sensitivity. Diagnosis was made based on these tests. Results From the analysis of ABR, DPOAE, TEOAE, and impedance data, 51 cases were found normal in the three tests. There were 64 cases with sensorineural hearing loss, 1 case with auditory neuropathy,2 cases with mixed hearing loss and 1 case with conductive hearing loss. Conclusion For infants and young children who failed the behavioral hearing test,it is a good way to adapted ABR and middle ear impedance measure, transient evoked otoacoustic emission (TEOAE),distortion product otoacoustic emission (DPOAE) to estimate their thresholds.
