血清CA_(125)和可溶性CD44标准型联合检测在恶性上皮性卵巢肿瘤诊断中的意义

摘　　要：目的研究联合检测血清可溶性CD44标准型(sCD44std)和CA125在恶性上皮性卵巢肿瘤诊断中的临床意义。方法采用定量酶联免疫吸附试验(ELISA)检测恶性上皮性卵巢肿瘤患者(49例)、良性上皮性卵巢肿瘤患者(34例)和正常妇女(80例)血清的sCD44std和CA125水平。结果恶性上皮性卵巢肿瘤患者的sCD44std水平显著高于良性上皮性卵巢肿瘤患者和正常对照组(P<0.05),良性上皮性卵巢肿瘤组患者与正常对照组的差异无显著性(P>0.05)。不同类型的恶性上皮性卵巢肿瘤sCD44std的差异无显著性(P>0.05)。sCD44std和CA125联合检测对恶性上皮性卵巢肿瘤诊断的灵敏度和正确率明显高于单项检测CA125或sCD44std(P值均<0.05)。国际妇产科联盟(FIGO)分期Ⅰ/Ⅱ期恶性上皮性卵巢肿瘤的血清sCD44std灵敏度高于CA125,但差异无显著性(P>0.05)。结论血清sCD44std和CA125联合检测可能提高恶性上皮性卵巢肿瘤的早期诊断率。Objective To determine the serum sCD44std and CA125 in the diagnosis of malignant epithelial ovarian tumor and analyze their clinical significances. Methods The level of serum sCD44std and and CA125 were measured by ELISA including epithelial ovarian malignant tumor （n= 49）, epithelial ovarian benign tumor （n=34）, normal ovarian tissues （n= 80）. Results ① Level of serum sCD44std： epithelial malignant tumor group was higher than that of the other two groups （P〈0.05）, there was no significant difference between the other two groups （P〉0.05）, also there was no significant difference among the different kinds of epithelial ovarian malignant tumor（P〉0.05） . ② Sensitivity and accuracy for the combination of of serum sCD44std and CA125 in epithelial ovarian malignant tumor ： the sensitivity and accuracy were higher than those of CA125 alone（P〈0.05）, or sCD44std alone （ P 〈 0.05）. ③ Sensitivity of epithelial ovarian malignant tumor （ stage Ⅰ/ Ⅱ ） ： sCD44std was higher than CA125, but there was no significant difference between the two groups（P〉0.05）. Conclusions The level of serum sCD44std together with CA125 can promote the diagnosis of malignant epithelial ovarian tumor. （Shanghai Med J, 2006, 29..95-97）
