硬腭黏骨膜侧翻瓣修复宽大腭裂的效果

摘　　要：目的探讨新的腭裂修复形式对腭裂缺损的修复效果。方法将一侧硬腭黏骨膜从外侧切开,形成以同侧裂隙边缘为蒂的黏骨膜侧翻瓣,向内侧翻转展开,修复腭裂鼻、咽腔面的缺损,另一侧腭粘骨膜瓣平移旋转覆盖其上;供修复完全性单侧以及双侧腭裂73例。结果所有接受该法的患者均达到一期愈合,无复裂发生。术后经鼻咽内窥镜检查,3个月时腭咽闭合功能完全和发音明显改善者64例;一侧软腭动度良好、腭咽闭合较好和发音有所改善者9例。结论硬腭黏骨膜侧翻瓣手术适用于完全性和不完全性腭裂患者,尤其对宽大腭裂患者的术后腭咽闭合功能及发音的改善效果明显。Objective To study a new method to repair broad cleft palate.Methods One side hard palate mueoperiosteum was incised from far-side and a side-reversed mucoperiosteal flap was made,which pediele located at the same side of cleft edge.The flap was reversed to inside for repairing pharyngonasal cleft. The other side mueoperiosteal flap was slid to cover the side-reversed one.Results 73 cases of complete unilateral or bilateral deft palate were repaired by above-mentioned method.All of them healed by first intervention without recleft.Three months later, pharyngorhinoscopy showed that complete palatopharyngeal close function and obviously improved pronunciation were obtained in 64 cases;wen unilateral soft palate movement and improved palatopharyngeal close function and pronunciation were observed in 9 cases. Conclusion Hard palate side-reversed mucoperiosteal flap is suitable for complete and incomplete cleft palate.Especially in broad cleft palate cases, their palatopharyngeal close function and pronunciation would be improved obviously.
